磷脂酰肌醇3-激酶p110β在胃癌中的表达及其与预后的关系

摘　　要：目的:研究磷脂酰肌醇3-激酶(PI3K)p110β蛋白在胃癌组织中的表达,并探讨其与预后的关系。方法:收集本院1996至2001年59例胃癌患者手术标本,应用PI3Kp110β多克隆抗体免疫组织化学方法检测其不同类型胃癌组织及癌旁正常胃组织中PI3Kp110β的表达。采用生存分析法分析其与预后的关系。结果:在59例胃癌组织中,PI3Kp110β阳性表达47例;在正常胃组织中其阳性表达13例(P<0.05)。肿瘤分化程度差的病例中PI3Kp110β阳性或强阳性表达明显高于肿瘤分化程度较好的病例(χ2=6.110,P=0.013);胃壁浸润较深(T3+T4)的病例中PI3Kp110β阳性或强阳性表达显著高于胃壁浸润浅(T1+T2)的病例(χ2=10.745,P=0.001);有淋巴结转移病例中PI3Kp110β阳性或强阳性表达高于无淋巴结转移的病例(χ2=4.511,P=0.034)。PI3Kp110β表达阳性或强阳性患者,其生存率显著低于PI3Kp110β表达弱阳性或阴性患者(P<0.01);且PI3Kp110β表达弱阳性患者其生存率也低于PI3Kp110β表达阴性患者(P<0.05)。结论:PI3Kp110β在低分化、浸润深及淋巴结转移的胃癌中高表达,PI3Kp110β的阳性表达对判断胃癌患者的预后有参考价值。Objective To study the expression of phosphatidylinositol 3-kinase （PI3K） P110β protein in gastric cancer and its correlation with prognosis. Methods Gastric cancer specimens of 59 patients undergone gastrectomy from 1996 to 2001 were collected. Multi-clone antibody immunohistochemistry staining was used to detect the expression of PI3KP110β in gastric cancer and paratumoral normal tissue specimens. Each batch had both positive and negative controls. Survival analysis was used to analyze its influence on prognosis. Results Of the 59 gastric cancer specimens, 47 were positive for PI3KP110β expression, while 13 paratumoral normal tissue specimens were positive （P〈0.05）. The positive- and strong positive- rates of PI3KP110β expression were much higher in specimens of poorly- differentiated than those of well-differentiated and moderately-differentiated 0（2=6.110,P=0.013）, and was significantly higher in cases with deep gastric wall invasion than those with superficial gastric wall invasion 0（2=10.745, P=-0.001 ）, and significantly higher in cases with lymph node metastasis than those without lymph node metastasis ff2=4.511 ,P=0.034）. Survival analysis found that the survival rate of cases with positive or strong positive PI3KP110β expression was much lower than cases with weak positive or negative PI3KP110βexpression （P〈0.01）. In addition , the survival rate of cases with weak positive PI3KP110β expression was lower than cases with negative PI3KP110β expression （P〈0.05）. Conclusions Overexpression of PI3KP110β was significantly correlated with differentiation , invasion and lymph node metastasis of gastric cancer, and might be an important prognostic factor.
