摘  要:目的探讨氢质子磁共振波谱分析(1H-MRS)在乳腺肿瘤中的临床价值及其影响因素。方法回顾性分析经手术或病理证实的58例乳腺肿瘤患者(乳腺癌35例,良性肿瘤23例)的1H-MRS特点,并与病理结果比较,评估1H-MRS诊断乳腺癌的价值。1H-MRS检查均在动态增强扫描后进行。结果 35例乳腺癌中有19例出现胆碱(Choline,Cho)峰,23例良性肿瘤中有5例出现Cho峰。以出现Cho峰为恶性肿瘤的鉴别标志,1H-MRS诊断乳腺肿瘤的敏感性为54.3%,特异性为82.6%,准确性为79.3%。结论 1H-MRS诊断乳腺癌的特异性较高,乳腺癌Cho峰检出率低与病灶和技术等因素有关。Objective To study the clinical values and influential factors of 1H magnetic resonance spectroscopy (1H-MRS) on breast tumors. Methods 1H-MRS features offivty- eight patients with breast tumors confirmed by pathology, including 35 cases of breast cancer and 23 cases of benign tumors, were retrospectively studied, and compared with pathologic results. The value of 1H-MRS on the diagnosis of breast cancer was evaluated. 1H-MRS were performed after dynamic enhanced scanning. Results In 35 breast cancers, 19 cases showed choline peak. In 23 benign tumors, 5 cases showed choline peak. Take choline peak as the diagnosis standard, the sensitivity, specificity and accuracy of 1H-M RS assess breast tumors were 54.3%, 82.6% and 79.3% respectively. Conclusion 1H-MRS has high diagnostic specificity to breast cancer. The low detection rate of choline peak of breast cancer relates to lesions and technique.
