氩氦刀靶向治疗联合TACE治疗乏血供肝癌的相关性研究

摘　　要：目的:探讨氩氦刀靶向冷冻联合选择性动脉化疗栓塞治疗乏血供肝细胞癌的方法和疗效。方法:对27例乏血供肝细胞癌患者利用氩氦刀进行冷冻治疗后行选择性动脉化疗栓塞治疗;另选25例乏血供肝细胞癌患者单纯行选择性动脉化疗栓塞治疗。两组病例治疗6～12个月后行CT或MRI复查,并对两组患者进行12～24个月随访。结果:联合治疗组12个月生存率为77.7%;24个月生存率为66.6%。6～12个月CT或MRI复查,病灶完全坏死者占13%;癌肿缩小1/2或3/1者为45%。单纯行选择性动脉化疗栓塞治疗组12个月生存率为60%;24个月生存率仅为28%。6～12个月经CT或MRI影像学检查癌肿明显缩小者仅为11%。结论:联合治疗组有效率明显高于单纯行选择性动脉化疗栓塞治疗组。氩氦刀冷冻治疗,择期行选择性动脉化疗栓塞治疗乏血供肝细胞癌是疗效确切的微创治疗手段,可提高患者1年或2年的生存率。Objective： To explore targeted argon-helium Cryoablation combined with selective arterial embolization for the treatment of spent blood supply patients with hepatocellular carcinoma. Methods： 27 cases of lack of blood supply to hepatocellular carcinoma were treated with cryosurgical cryotherapy to conduct selective arterial chemoembolization. Another 25 cases simply received selective arterial chemoembolization. CT or MRI was used for re-examination in the 2 groups 6-12 months later. All the patientswere followed up for 12-24 months. Results： Combined treatment of groups of 12-month survival rate of 77.7%, 24 month survival rate of 66.6%, CT or MRI review 6-12 months, focus complete necrosis of them, 13%, cancer by narrowing or one-second for 45%. Selective arterial chemotherapy and embolization in the treatment of simple group 60% 12-month survival rate, 24 month survival rate is only 28%, 6-12 months by CT or MRI Imaging cancer significantly reduce only to 11%. Conclusion： Combined treatment group effective rate significantly higher than single selective arterial infusion chemotherapy and embolization in the treatment group. First Cryoablation in treatment, selective intraarterial chemoembolization for lack of blood supply of liver cell carcinoma is a definite effect of minimally invasive treatments, it can greatly improve the 1-year or 2-year survival rate.
