摘 要:目的应用表面增强激光解析离子化飞行时间质谱(SELDI-TOF-MS)技术检测食管鳞状细胞癌患者血清蛋白指纹图谱,分析特异相关性蛋白与临床病理因素的关系。方法 144例经手术后病理证实的食管鳞状细胞癌患者,用弱阳离子交换芯片(WCX2)检测各血清标本获得血清蛋白指纹图谱,用Biomarker Wizard筛选差异峰;比较不同年龄、临床分期、淋巴结转移及食管外膜侵犯各组差异有统计学意义的蛋白峰。结果年龄40~60岁(77例)和>60岁(67例)患者表达的蛋白差异峰差异无统计学意义(>0.05)。28例I期与51例III期患者比较发现10个差异峰差异有统计学意义(<0.05);28例I期与6例IV期患者比较发现1个差异峰差异有统计学意义(<0.05)。而28例I期与59例II期患者比较,未发现差异峰差异有统计学意义(>0.05)。69例淋巴结转移和75例无淋巴结转移患者比较,发现5个差异峰差异有统计学意义(<0.05)。53例无外膜浸润和91例有外膜浸润患者比较,发现有9个差异峰差异有统计学意义(<0.05)。结论应用SELDI-TOF-MS技术检测分析食管鳞状细胞癌患者血清蛋白指纹图谱,对个体化治疗、预后判断有一定的预测提示作用。Objective To detect the serum proteomic patterns in patients with esophagus squamous cell carcinomas by using SELDI-TOF-MS, and to analysis the correlations between proteomic patterns and clinical pathological factors. Me- thods SELDI-TOF-MS was used to detect the serum proteomic patterns of 144 patients with esophagus squamous cell carcinomas. Biomarker Wizard software was used to detect protein peaks significantly different between esophageal can- cer and controls. The proteomic analysis was also performed on serum from esophageal patients according to deferent age, clinical stages, lymph node metastasis, and esophagus serosa invasion. Results No significantly deferent protein peak was found between 40 ~ 60 years and 〉 60 years group(P 〉 0.05). There were ten significantly deferent protein peaks detected between stage I and stage III (P 〈 0.05). There were one significantly deferent protein peaks found be- tween stage I and stage IV (P 〈 0.05). But no significantly deferent protein peak was detected between stage 1 and stage II (P 〉 0.05) .There were five different protein peaks between 65 cases patients with lymph nodes metastasis and 75 cases without metastasis (P 〈 0.05). Nine different protein peaks were verified to discriminate patients with and without esophagus serosa invasion (P 〈 0.05). Conclusions SELDIT-OF-MS can be regarded as an effective method for esophageal neoplasm diagnosis and prognosis indication.
