强迫症患者自知力及对选择性5-羟色胺再摄取抑制剂的治疗反应性

摘　　要：目的探讨强迫症患者自知力及对选择性5-羟色胺再摄取抑制剂(SSRIs)的治疗反应性。方法强迫症患者64例,用耶鲁-布朗强迫量表(YBOCS)第11条评价患者自知力,然后将患者分为自知力较好组和自知力较差组,两组患者均用SSRIs治疗,分别于治疗前、治疗后第12周用YBOCS、HAMD及HAMA量表进行评定,并进行对比分析。结果根据YBOCS第11条自知力得分,其中自知力较好组(<3分)有39例(60.9%),自知力较差组(≥3分)有25例(39.1%),两组治疗前后自知力较差组YBOCS、HAMA、HAMD评分均高于自知力较好组(P<0.05),治疗后自知力较好组有效率高于自知力较差组(P<0.05),治疗后患者自知力评分低于治疗前(P<0.05)。结论强迫症患者中有一部分患者自知力较差,与自知力较好的患者相比自知力较差的患者症状较重,对SSRIs治疗效果较差。治疗后患者自知力有所改善。[Abstract] Objective To explore the insight and the treatment response to selective serotonin reuptake inhibitors （SSRIs） in patients with obsessive-compulsive disorder （OCD）. Methods 64 patients with OCD were assessed with item 11 of YBOCS to evaluate their insight. Then these patients were divided into good insight guoup and poor insight group. All patients were treated with SSRIs and were assessed with YBOCS, HAMD and HAMA at baseline and at the 12nd weekend of treatment. Results According to the score of item 11 of Y-BOCS, 60.9% of the patients （39 cases） had good insight and （25 cases） （39.1% had poor insight. Scores of YBOCS, HAMD and HAMA were higher in the poor insight group than those in the good insight group at baseline and at the end of the treatment （ P 〈 0.05）. The effective power was higher in good insight group than that in the poor insight group at the end of the treatment （P 〈 O. 05）. Score of insight was lower at the end of the treatment than that at baseline （ P 〈 O. 05 ）. Conclusion Some patients with OCD may have poor insight, which have more serious symptoms and poor treatment response to SSRIs when compared with OCD patients with good insight. Insight in patients with OCD can be improved after the treatment of SSRIs.
