复方苦参注射液联合顺铂胸腔内灌注治疗恶性胸腔积液近期疗效观察

摘　　要：目的探讨胸腔置管引流术并腔内灌注复方苦参注射液联合顺铂治疗恶性胸腔积液的近期疗效与不良反应。方法经病理学或细胞学确诊为恶性胸腔积液患者共78例，按入院先后顺序随机分为观察组与对照组，每组各39例。在充分引流胸腔积液后，观察组胸腔内注人复方苦参注射液25ml＋顺铂40-60mg，对照组胸腔注入顺铂40-60mg，2组均每周1次，共计4次，观察疗效、生活质量及不良反应。结果观察组与对照组总有效率分别为84．6％（33／39）和59．0％（23／39），组间比较差异有统计学意义（P〈0．05）。观察组用药后生活质量改善明显，卡氏评分提高，改善率为74．4％（29／39），明显高于对照组的46．1％（18／39），组间比较差异有统计学意义（P〈0．05）。观察组各种不良反应发生率均低于对照组，组间比较差异有统计学意义（P〈0．05）。结论胸腔置管引流并腔内灌注复方苦参注射液和顺铂治疗恶性胸腔积液疗效优于单用顺铂，且可提高患者生活质量、减少不良反应的发生。Objective To investigate the recent curative and side effects of using compound kushen injection and cisplatin as pleural infusion for malignant pleural effusion. Methods Seventy-eight patients who were diagnosed of malignant pleural effusion based on pathology and cytology were divided into two groups randomly. Observation group： compound kushen injection 25 ml ＋ 0. 9% sodium chloride injection 40 ml ＋ cisplatin 40-60 ml were injected into thoracic cavity after adequate drainage. Control group： 0. 9% sodium chlorideinjection 40 ml ＋ cisplatin 40-60 ml were injected into thoracic cavity after adequate drainage. The curative effects, quality of life and side effects were recorded. Results In the observation group, 14 patients were complete remission, 19 patients were partial remission and 6 patients had no response. The total response rate was 84.6% （33/39）. In the control group, 11 patients were complete remission, 12 patients were partial remission and 16 patients had no response. The total response rate was 59.0% （23/39）. The total response rate in the observation group was obviously higher than that in the control group （P 〈 0. 05 ）. The improvement of quality of life in observation group [ 74. 4% （29/39） ] was obviously higher than in the control group [46. 1% （18/39） ] （P 〈0. 05）. The incidence rate of side effects in observation group was much lower than that in the control group （ P 〈 0. 05 ）. Conclusions For malignant pleural effusion, using compound kushen injection with cisplatin as pleural infusion is better than using cisplatin only. It can also improve the patients＇quality of life and prevent the incidence of the side effects.
