VEGF诊断原发性肝癌准确性的Meta分析及系统评价

摘　　要：目的通过Meta分析及系统评价评估血管内皮生长因子(VEGF)对原发性肝癌的诊断价值。方法通过计算机检索及手工查询获得VEGF诊断原发性肝癌的文献,通过纳入及排除标准筛选文献并提取数据,采用MetaDisc1.4软件检验研究间异质性,并对诊断准确度指标进行Meta分析,绘制综合受试者工作特性曲线(SROC)。结果 VEGF诊断原发性肝癌的汇总灵敏度、特异度及诊断优势比分别为0.70(95%CI:0.67～0.73)、0.86(95%CI:0.84～0.87)、17.27(95%CI:11.32～26.33)。SROC曲线下面积(AUC)为0.8921,Q指数为0.8221。结论 VEGF在原发性肝癌的诊断中具有较好的灵敏度及准确性,可作为诊断原发性肝癌的重要指标。Objective To assess the diagnostic value of serum vascular endothelial growth factor（VEGF） for primary liver cancer.Methods Literatures about researches on the clinical usage of VEGF for the diagnosis of primary liver cancer were retrieved by using computer or manually sought,and data were extracted from enrolled literatures.Heterogeneity between enrolled literatures were analyzed by using Meta Disc1.4 software,and the diagnostic accuracy was analyzed by Meta analysis and receiver operating character（ROC） curve.Results Pooled sensitivity,specificity and diagnostic odds ratio（DOR） were 0.70（95%CI：0.67-0.73）,0.86（95%CI：0.84-0.87）and 17.27（95%CI：11.32-26.33） respectively.The area under ROC curve（AUC） and Q index were 0.892 1 and 0.822 1.Conclusion VEGF might have reasonable sensitivity and specificity,and could play an important role for the diagnosis of primary liver cancers.
