高尔基体蛋白GP73在肝癌诊断中的临床意义

摘　　要：目的定量检测样本血清中高尔基体蛋白73(GP73)的水平,同时对比GP73与甲胎蛋白(AFP)的检出,探讨GP73用于肝癌诊断的临床意义。方法收集肝癌患者、肝炎及肝硬化、其他癌症以及健康人员样本,分别检测GP73和AFP,GP73检测采用酶联免疫定量测定,AFP检测用电化学发光法。结果在186例肝癌标本中,GP73阳性率76.34%,AFP阳性率41.94%,经过χ2检验,两者差异有统计学意义(χ2=22.78,P<0.01);在肝炎及肝硬化患者和其他肿瘤患者中的GP73阳性检出率分别为10.66%和6.41%;AFP的阳性检出率分别为26.23%和26.92%,经过χ2检验,两者的检出率差异具有统计学意义(P<0.05);在健康人员中,GP73和AFP的检测特异性均为100%;ROC曲线分析显示,GP73和AFP的曲线下面积分别为0.932和0.770。结论 GP73用于诊断肝癌效果明显优于AFP,可以显著提高对肝癌诊断的灵敏度和特异性,而且GP73相比AFP更不容易受到肝部炎性反应和其他部位肿瘤的影响。Objective To investigate the clinical significance of the detection of serum level of Golgi glycoprotein 73（GP73） for the diagnosis of liver cancer.Methods GP73 was quantitatively analyzed by using enzyme linked immunosorbent assay（ELISA） and alpha fetoprotein（AFP） was quantitatively detected by using Electrochemiluminescence immunoassay（ECLI）.Results There were significant differences in the positive rate between GP73（76.34%） and AFP（41.94%） in the serum of patients with liver cancer（χ2=22.78,P〈0.01）.There were significant differences in the positive rate between the GP73（10.66%,6.41%） and AFP（26.23%,26.92%） in the serum of patients with hepatitis and hepatocirrhosis and patients with other tumour（P〈0.05）.No significant difference between the detectable rate of GP73 and AFP was found in the serum of healthy subjects.ROC curve analysis showed that the area under the curve of GP73 and AFP were 0.932 and 0.770,respectively.Conclusion GP73 might be superior for the diagnosis of liver cancer than AFP,and GP73 could significantly improve the diagnostic sensitivity and specificity of liver cancer.GP73 might be less susceptible than AFP for the influencing of liver inflammation and tumors of other parts.
