云南省富源地区肺癌危险因素的Meta分析

摘　　要：目的云南省富源县是中国肺癌高发地区之一，探讨该地区人群肺癌发生的危险因素，为肺癌的预防控制提供科学依据。方法通过Pubmed、Webofknowledge、Google、中国期刊全文数据库和万方数据资源系统等检索1995年1月至2011年12月公开发表的关于该地区人群肺癌发病危险因素的国内外研究文献，采用RevMan4．2软件进行Meta分析。结果共纳入12篇文献，累计病例4666例，对照5298例。各因素合并比值比分别为生活用煤（4．40）、居室通风情况（2．15）、炼煤环境（2．02）、吸烟（1．72）、女性被动吸烟（1．70）、易感基因（肿瘤家族史）（1．65）、慢性呼吸道疾病（1．52）、饮食习惯（饮酒和高脂饮食）（1．39）、蔬果摄人多（0．65）。结论影响富源地区人群肺癌发生的危险因素为室内外环境污染、吸烟、家族史、呼吸系统疾病和饮食习惯等，提示环境因素和遗传因素在该地区肺癌的发病中有重要作用。Objective Fuyuan is one of the areas with highest prevalence of lung cancer in China. The present study analyzed the main risk factors for high incidence of lung cancer in Fuyuan to direct future lung cancer prevention. Methods We reviewed previous studies on lung cancer in Fuyuan of Yunnan province from January 1995 to December 2011. by searching Pubmed, Web of knowledge, Google, Chinese National Knowledge Infrastructure and ChinaInfo. The Meta analysis was done using RevMan 4.2. Results A total of 12 studies involving 4 666 patients and 5 298 controls were included. The pooled odd ratio （OR） values were 4.40 （indoor coal pollution）, 2.15 （ventilation in kitchen）, 2.02 （exposure to coal-mine production）, 1.72 （smoking）, 1.70 （second hand smoke for women）, 1.65 （genetic susceptibility, family history）, 1.52 （certain pulmonary diseases）, 1.39 （diet, cholesterol, alcohol）, and 0.65 （vegetables and fruits）. Conclusions The main risk factors for lung cancer are environmental pollution, smoking, family history, certain pulmonary diseases and diet. Environmental pollution and genetics contribute to development of lung cancer in Fuyuan of Yunnan province in China.
