软肝化坚颗粒抗肝纤维化作用的临床研究

摘　　要：目的研究软肝化坚颗粒的抗肝纤维化作用。方法将200例慢性乙型病毒性肝炎(以下简称乙肝)肝纤维化患者随机分为2组。治疗组100例予益气健脾、活血化瘀复方制剂软肝化坚颗粒治疗,对照组100例予复方鳖甲软肝片治疗。2组均治疗6个月。观察2组治疗前后肝纤维化血清标志物、肝功能,并采用肝脏彩色多普勒超声观察血流动力学指标。结果 2组治疗后肝纤维化血清标志物水平均较本组治疗前显著降低(P<0.05)。2组治疗后肝功能丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)及总胆红素(TBiL)均较本组治疗前降低(P<0.05),血清白蛋白(Alb)升高(P<0.05)。2组治疗后血流动力学指标均较本组治疗前显著改善(P<0.05)。结论软肝化坚颗粒具有确切的抗肝纤维化及保肝作用。Objective To explore the anti-fibrosis effect of Ruanganhuajian granule in patients with chronic hepatitis B.Methods Two hundred patients with chronic hepatitis B were divided into two groups,i.e.treatment group and control group.There were 100 cases in every group.The treatment group was treated with Ruanganhuajian granule with the effects of replenishing qi to invigorate the spleen and promoting blood circulation to remove blood stasis.The control group was treated with Fufang biejiaruangan tablets.The serological parameters of hepatic fibrosis,liver function and Color Doppler scan were detected before the treatments started and after the patients had been treated for 24 weeks.Results The levels of serological parameters of hepatic fibrosis including laminin,hyaluronic acid,collagen type Ⅳ and procollagen type Ⅲ were significantly lower than those before treatment（P〈0.05）;Liver functions including alanine aminotransferase（ALT）,aspartate aminotransferase（AST）,total bilirubin（TBiL） and serum albumin were significantly ameliorated（P〈0.05）.Serum albumin after treatment were significantly ameliorated（P〈0.05）.The hemodynamic indexes of Color Doppler scan after treatment were better than those before treatment（P〈0.05）.Conclusion Ruanganhuajian granule with the effects of replenishing qi to invigorate the spleen and promoting blood circulation to remove blood stasis have significant effects on hepatic fibrosis and liver function in patients with chronic hepatitis B.
