宫颈癌低氧放疗的疗效分析

摘　　要：目的　探讨宫颈癌低氧放疗的临床应用价值 .方法　 1995 - 0 2 / 1998- 0 6 ,80例宫颈癌随机分为低氧放疗组和常规放疗组各 40例 .两组均采用腔内照射与外照射相结合的方法 .腔内照射剂量 40～ 5 0 Gy/ 8～ 10 wk,体外照射剂量 5 0Gy/ 6～ 8wk.低氧组佩戴低氧呼吸装置 (相当于呼吸含氧量为 10 .5 %的低氧气体 ) .结果　低氧组和常规放射治疗组的肿瘤完全缓解 (CR)、部分缓解 (PR)、无效应 (NR)分别为6 0 .0 %和 6 2 .5 % ,35 .0 %和 32 .5 % ,5 .0 %和 5 .0 % ;直肠炎、膀胱炎、阴道粘连、厌食、恶心或呕吐、腹泻的发生率分别为2 .5 %和 15 .0 % ,0 %和 7.5 % ,12 .5 %和 45 .0 % ,5 .0 %和32 .5 % ,2 .5 %和 12 .5 % ,37.5 %和 45 .1% ;白细胞数变化分别为 (3.9± 0 .8)× 10 9· L- 1 和 (2 .8± 10 .7)× 10 9· L- 1 .结论　低氧放疗可减轻宫颈癌患者的放疗副作用和降低并发症的发生率 .AIM To evaluate the hypoxyradiotherapy in the treatment of cervical cancer. METHODS From Feb. 1995 to Jun. 1998, 80 patients with cervical cancer were randomized into two groups, hypoxyradiotherapy group (study group with 40 patients) and radiotherapy alone group (con-trol group 40 patients). All patients were given external-beam pelvic irradiation combined with intracavitary application. The dose of intracavity irradiation was 40~50 Gy/8~10 weeks, that of external-beam irradiation dose 50 Gy/6~8 weeks. The patients in study group inhaled the gas which contained oxygen in concentration of 10.5%. RESULTS Complete response (CR), partial response (PR) and no response (NR) were 60.0%, 35.0% and 5.0% in study group, 62.5%, 32.5% and 5.0% in control group respectively. The rate of rectitis, cystitis, vaginal conglutinations, anorexia, nausea or vomit and diarrhea was 2.5%, 0%, 12.5% , 5.0%, 2.5% and 37.5% in study group, 15%, 7.5% , 55.0%, 32.5%, 12.5% and 45.1% in control group respectively. Leukocyte counts were (3.90±0.80)×10 9·L -1 and (2.80±10.7)×10 9·L -1 in study and control groups respectively. CONCLUSION Our clinical trial indicates that the hypoxyradiotherapy of cervical cancer could decrease the rate of acute side-effects and treatment complications.
