膀胱移行细胞癌中Survivin的表达及临床意义

摘　　要：目的探讨凋亡抑制因子Survivin在膀胱移行细胞癌(BTCC)中的表达情况及其与BTCC病理临床特征的关系。方法 68例膀胱移行细胞癌石蜡包埋标本作为实验组,另取10例非肿瘤患者的正常膀胱黏膜组织作为对照组。采用免疫组化SP法检测所有标本中Survivin的表达,并进行统计学分析。结果 BTCC组、正常组Survivin的阳性表达率分别为72.1%、0%(P<0.05)。病理Ⅰ级、Ⅱ级、Ⅲ级者Survivin的阳性表达率分别为45.8%、83.3%、92.9%,Ⅰ级与Ⅱ级、Ⅲ级比较,差异均有统计学意义(P<0.05)。浅表型、浸润型,淋巴结阴性、阳性,初发、复发者Survivin的阳性表达率分别为57.9%、90.0%,64.7%、94.1%,57.5%、92.9%,组间比较,P均<0.05。结论 Survivin的异常表达在BTCC的发生、发展中起重要作用。BTCC组织中Survivin表达的检测对判断BTCC的发生发展、转移、复发及预后有一定帮助。Survivin也可能为BTCC基因治疗提供新的靶点。Objective To study the expression of Survivin in bladder transitional cell carcinoma（BTCC） and the relationship between its expression and the clinicopathological characteristics of BTCCs.Methods 68 cases of paraffin-embeded specimens of BTCC was taken as the experimental group and ten cases of non-tumor patients with normal bladder mucosa as a control group.Using immunohistochemical SP method to detect the expression of Survivin in all specimens,and the results were statistically analyzed.Results The positive rate of Survivin were 72.1% in the BTCC group and 0% in the normal group,respectively（P〈0.05）.The positive rate of Survivin in pathological grade Ⅰ,Ⅱ and Ⅲ were respectively 45.8%,83.3% and 92.9%.There was statistically significant difference between gradeⅠand gradeⅡ,gradeⅢ（P〈0.05）.That of Survivin in superficial,invasive;primary,recurrence;lymph node postive,negetive were 57.9%,90.0%;64.7%,94.1%;57.5%,92.9%;respectively（P〈0.05）.Conclusion The abnormal high expression of Survivin may play an important role in occurrence and progress of BTCC.Detection of Survivin in BTCC is helpful in evaluating the occurrence,progress,metastasis,relapase of BTCC and the prognosis of the patients.Survivin also can provide a new appropriate target for gene therapy to BTCC.
