UroplakinⅢ与Twist在膀胱尿路上皮癌中的表达及临床意义

摘　　要：目的探讨UroplakinⅢ(UPⅢ)、Twist在膀胱尿路上皮癌中的表达及临床意义。方法采用免疫组化SP法检测UPⅢ和Twist的表达。结果 UPⅢ在140例膀胱尿路上皮癌组的表达阳性率为15.7%(22/140)低于70例正常膀胱粘膜组71.4%(50/70),差异有统计学意义(P<0.05)。Twist在膀胱尿路上皮癌组的表达阳性率为72.9%(102/140)高于正常膀胱粘膜组6.0%(4/70),差异有统计学意义(P<0.01)。UPⅢ表达缺失与膀胱尿路上皮癌高分化,肌层浸润,淋巴管浸润呈正相关(P<0.05)。Twist阳性率与肿瘤分化程度,临床分期,淋巴管浸润有相关性(P<0.05)。两者均与患者年龄、性别无相关性(P>0.05)。结论 UPⅢ表达缺失及Twist的阳性表达均与膀胱尿路上皮癌的病理分级,TNM分期及淋巴管浸润有相关性,联合检测两者对诊断及评估预后膀胱尿路上皮癌有实用价值。Objective To explore the expression and clinical significance of Uroplakin Ⅲ（UPⅢ） and Twist in the patients with bladder urothelial cancer.Methods The UPⅢ and twist expressions in 70 cases of bladder urothelial cancer and 35 cases of normal bladder mucous membrane were detected by SP immunohistochemistry,and their clinical and pathological characteristics were analyzed.Results The expression of the positive rate of UPⅢ in patients with bladder urothelial cancer（15.9%） was significantly lower than that in normal bladder mucous membrane（71.4%）.The positive rate of Twist expression in bladder mucous membrane（72.9%） was significantly higher than that in moral bladder mucous membrane（6.0%）.The decreased expression of UPⅢ was associated with high-grade of urothalial carcinoma,muscle-invasive cancer and lymphovascular invasion（P0.05）.The positive rate of Twist showed a positive correlated in tumor differentiation,staging and lymphovascular invasion（P0.05） and no correlation with ages and gender（P0.05）.Conclusion The decreased expression of UPⅢ and the positive expression of Twist are positively correlated with histopathologic grade of urothalial carcinoma,TNM stage and lymphovascular invasion.It may be reflect practical value in both diagnosis and assessment of prognosis.
