摘  要:目的研究本体感觉训练对脑卒中偏瘫患者运动功能及日常生活活动能力的影响。方法将脑卒中偏瘫患者40例按随机数字表法分为治疗组(20例)和对照组(20例)。2组患者均给予常规康复治疗及药物治疗,治疗组在此基础上辅以本体感觉训练。2组患者均于治疗前和治疗4周后(治疗后)采用Tecnobody本体感觉系统评估、Fugl—Meyer下肢运动功能评定(FMA)、Berg平衡量表(BBS)、改良Barthel指数评定表(BI)分别评估其本体感觉、下肢运动功能、平衡能力及日常生活活动能力。结果治疗后,治疗组患者本体感觉能力、下肢运动功能、平衡能力及日常生活活动能力均较治疗前有明显提高(P〈0.05);对照组患者下肢运动功能、平衡能力及日常生活活动能力均较治疗前有明显提高(P〈0.05),但本体感觉能力较治疗前末见明显改善(P〉0.05);组间比较发现,治疗组平衡能力及日常生活活动能力的改善优于对照组(P〈0.05)。结论本体感觉训练能更好地改善脑卒中偏瘫患者的运动功能(包括本体感觉、平衡功能)及日常生活活动能力。Objective To study the effects of proprioception training on the motor function and ability in the activities of daily living (ADL) of hemiplegie stroke patients. Methods Forty hemiplegic stroke patients were divided into study group and a control group (n = 20 in each). The two groups received conventional rehabili- tation treatment and pharmacotherapy, but the study group received additional proprioception training. The Techno- body proprioeeptive system assessment, the Fugl-Meyer lower limb assessment (FMA) , the Berg balance scale (BBS) and the modified Barthel index (MBI) were used to evaluate their proprioceptive ability, lower limb motor fllnction, balance and ADL ability, before and after 4 weeks of treatment. Results After 4 weeks, propriocep- tion, lower limb motor function, balance and ADL ability had all improved significantly in the experimental group. Lower limb motor function, balance and ADL ability had also improved significantly in the control group, but they showed no significant change in their average proprioeeptive ability. The improvement in the experimental group was superior to that of the control group in terms of proprioceptive ability, balance and ADL ability. Conclusions Proprioception training can improve the motor function ( including the proprioception and balance and ADL ability of hemiplegie stroke patients.
