Observation the Therapeutic Effect of Multiple Amniotic Membrane Transplantation on Severe Ocular Alkali Burn in Early Stage

摘  要:目的:观察重度眼部碱烧伤早期经多次羊膜移植治疗的临床效果。方法:回顾性分析我院2008年1月~2011年6月12例(18只眼)重度眼部碱烧伤患者,伤后采用早期重复羊膜移植术直至上皮缺损和溃疡愈合。术后观察角膜透明度、上皮修复及视力等进行观察随访。随访5—8个月,平均6.5个月。结果:18眼术后无排斥反应和激发感染发生,眼表迅速稳定,角膜不同程度恢复透明,18眼视力不同程度提高,4眼矫正视力恢复到0.02、8眼矫正视力0.02。0.1,5眼矫正视力0.1-0.5,1眼好于眼前数指。结论:早期多次新鲜羊膜移植能有效阻止眼部重度碱烧伤后的炎症、睑球粘连和角膜融解的发展,加快眼表的重建,减轻瘢痕,不失为治疗严重的眼部碱烧伤的一种简单可行、实用有效的方法。Objective:To analysis of therapeutic effect of 12 cases ( 18 eyes) severe ocular alkali burn patients with early multiple amniotic membrane transplant treatment in our hospital from Jan 2008 to Jun 2011. The ocular surface stability retrospective, epithelial healing, transparency of the cornea, corneal neovascularization and visual activity were observed during 5-8 months follow-up. Results: There w was no allograft immune rejection or secondary infection in 12 cases( 18 eyes). The ocular surface rapidly stability .Corneal different degree restore transparent . In 12 cases(18eyes) Visual acuity improved to varying degrees. 4 eyes were 0.02 in visual acuity, 8 eyes were 0.02-0.1, 5 eyes were0.1-0.5 and leye was more than finger-nummber. Conclusions: Multiple amniotic membrane transplantation ( especially using a fresh membrane) could effectively reduce the inflammation of the cornea at the acute stage of burn injury, can prevent corneal melting of development,and symblepharon. It could make the reconstruct process faster for the ocular surface , and effectively reduce fibrosis. Early amnioticm embrane transplantation for severe ocular alkali burns might be a smiple and feasible,practical and effective method.
