上颌前牙早期种植的临床应用

摘　　要：目的探讨上颌前牙缺牙区行早期种植的效果及技术要点。方法对22例上前牙病变患者行微创拔牙,术后4～8周共植入种植体28枚,其中10例伴骨缺损者采用GBR技术,同期植入人工骨粉及胶原膜,严密关闭切口,3～6个月后行永久修复。随访时间6～27个月(平均12个月)。在此期间检查种植体稳固性,牙龈及龈乳头外形;X线检查种植体骨结合情况;评价临床成功率及美学修复效果。结果 28枚种植体存留率为100%。牙龈形态良好,色泽正常,无明显探诊出血,与对侧同名牙牙龈形态基本对称。复查X线片种植体颈部边缘骨吸收均小于1.5 mm。结论上前牙早期种植应用于临床方法可行,修复缺牙美学效果良好。Objective To evaluate the clinical efficacy and technical points of early implantation in the anterior maxilla. Methods Twen- ty-two patients with 28 teeth extracted in anterior maxilla were treated with XIVE implants 4 - 8 weeks after extraction. Guided bone regenera- tion was performed in 10 implant sites. After healing for 3 -6 months, final prostheses were performed. The time in function of the implants was 12 months （6 -27 months）. The stability of the implants, the shape of gingiva and gingival papilla were assessed and the X-ray was used to evaluate the clinical outcomes of early implant placement. Results The survival rate of the implants was 100% in observation period. All of the implants had healthy and well-shaped gingiva, and the mean loss in margin bone height was less than 1.5ram. Conclusions The re- sults demonstrated that early implantation could have successful tissue integration and pleasing esthetic outcomes.
