“立减递增提速法”白细胞去除术治疗高白细胞性急性白血病

摘　　要：目的:探索"立减递增提速法"白细胞去除术治疗高白细胞性急性白血病的白细胞清除作用及安全性。方法:白细胞去除术一开始立即将全血流速减低为20ml/min,每隔5min将全血流速增加10ml/min,直至达到机器自动运行的流速。开始采集白细胞时,将速率提升至5-10ml/min。结果:"立减递增提速法"治疗前后白细胞计数比较有统计学意义,与传统方法比较,低血钙和低血容量症状发生率下降,去除时间缩短,病人在CR、PR、NR、早期死亡率方面比较无统计学意义。结论:"立减递增提速法"白细胞去除术清除白细胞的效果与传统去除方法相同,不良反应低血钙、低血容量反应发生率下降,去除时间缩短,值得临床推广应用。Objective： To investigate the efficacy and safety of improved - leukocytapheresis in the treatment of hy-per-leukocytic leukemia（HLL）. Methods. HLL patients were treated with the improved-leukocytapheresis, com-pare with HLL patients treated with traditional leukocytapheresis. Compare the differences in efficacy and safety be-tween the two kinds of treatments. Results： WBC counts decreaced remarkably after improved - leukocytapheresis treatment （P 〈 O. 05）. Incidences of hypo - calcium and hypo - volume were lower in improved - leukocytapheresis. There were no significant differences in CR,PR, NR and early mortality between 2 methods. Conclusion： Improved-leukocytapheresis had the same effect as traditional leukocytapheresis on leukocyte depletion, meanwhile, it re-duced the adverse reactions, and shortened the operating time. This method is worthy of being popularized.
