中央角膜厚度与正常眼压性青光眼的临床关系

摘　　要：目的:探讨正常眼压性青光眼(NTG)患者中央角膜厚度(CCT)与眼内压(IOP)变化之间的关系,明确CCT在NTG疾病诊断及治疗中的指导意义。方法:对2006年至2011年间临床诊断为正常眼压性青光眼患者眼内压变化与中央角膜厚度进行前瞻性研究。评价治疗前后平均CCT和IOP之间的变化关系。NTG诊断标准为典型的视野缺损,青光眼视野改变及眼压<22mmHg。结果:CCT显示与青光眼严重程度呈负相关。在轻度NTG病例,CCT结果正常,中度NTG病例中CCT数值较小,重度NTG病例中CCT测量结果明显降低。IOP结果在青光眼发展各阶段均无明显变化,但治疗后各阶段眼压降低程度与疾病进展程度相关。依疾病进展程度不同,在轻、中、重度NTG患者,治疗后双眼平均IOP降低分别为13.34,12.41,11.87mmHg。结论:CCT与NTG进展程度相关,NTG进展越严重,CCT结果越低。IOP与NTG进展程度无关,但严重NTG病例经药物干预后IOP降低程度小于轻、中度病例,提示对于严重NTG病例可能需强化药物治疗或及时手术干预。Objective： To correlate central corneal thickness（CCT） and intraocular pressure（IOP） with disease severity in normal-tension glaucoma（NTG） patients.Method： We conducted prospective Studies of all patients diagnosed with NTG between 2006 and 2011.NTG was diagnosed according to the glaucomatous visual fields loss,glaucomatous optic disc cupping,and an IOP 22mmHg on diurnal curve measurements.Mean CCT and IOP values before and after treatment were also evaluated.Patients were divided into 3 groups according to advanced glaucoma intervention score（mild,moderate,and severe visual field defects）.Result： A total of 13 males and 11 females with bilateral NTG were enrolled.The mean follow-up was 3.35 years.CCT was inversely correlated with glaucoma severity.CCT was normal in both eyes in mild disease,thin in moderate disease,and low in both eyes in severe disease.Initial bilateral mean maximal IOP was similar at all disease stages and became lower after treatment in parallel to disease severity： 13.34,12.41,11.87mmHg respectively.Conclusion： CCT correlated with disease severity： the more advanced the disease,the thinner the cornea.Initial maximal IOP did not predict disease severity,but it was lower in the less severe cases after treatment,possibly representing a more aggressive treatment protocol.
