p53、MDM2在低级别胶质瘤复发、恶变中的变化及意义

摘　　要：目的低级别胶质瘤的预后明显好于高级别胶质瘤。但部分低级别胶质瘤术后很快复发，且伴有肿瘤级别的增加。本研究旨在探讨p53、MDM2基因在初发低级别及复发、恶变后高级别胶质瘤中表达的差异并分析这种变化的意义。方法采用免疫组化S-P二步法对24对初发低级别及复发、恶变后高级别胶质瘤标本分别进行p53、MDM2的表达检测，分析p53和MDM2之间的相关性，并对它们在低级别胶质瘤复发、恶变过程中的表达差异进行统计分析。结果与初发的低级别胶质瘤标本相比较，复发、恶变的胶质瘤中p53、MDM2的表达明显升高，其表达差异有统计学意义（P〈0．05）。本组资料中053与MDM2的表达存在正相关性（r=0．695，P〈0．01）。结论本研究结果表明，在低级别胶质瘤复发、恶变中p53、MDM2起到了重要的作用。p53、MDM2的表达可以做为低级别胶质瘤预后较差和复发恶变的重要指标。Objective The prognosis of the patients with low grade glioma is better than those with high grade glioma. But some of the patients with low grade glioma reccur in a short time after operation, following by the grade of the tumors increased. This study was to detect the difference and significance of p53 and MDM2 expres- sions in the patients with low grade glioma and recurrent and malignant development. Methods The expressions of p53 and MDM2 in 24 patients with low grade gliomas were detected and tested also with recurrent and malig- nant development by immunohistochemistry SP method. The relationship between p53 and MDM2 expressions of the patients was also analyzed. The difference and significance of p53 and MDM2 expression in the patients with low grade glioma and recurrent and malignant development were also investigated. Results The expressions of p53 and MDM2 were significantly increased in gliomas with recurrent and milignant development compared with low grade glioma （P 〈 0.05 ）. The relavance between p53 and MDM2 expression was positive in this study;related coefficient was 0.665. Conclusion It was proved that the expressions of p53 and MDM2 played an important role in the recurrent and milignant development of low grade glioma. The expressions of p53 and MDM2 may be used as a indicator of a poor prognosis and recurrent and malignant development in low grade glioma.
