牙周炎龈沟液中IL-6、IL-10水平变化与临床指标关系的研究

摘　　要：目的:探讨牙周炎病人牙周基础治疗前及治疗后6wk、8wk、10wk龈沟液(gingival crevicular fluid,GCF)中白细胞介素-6(interleukin-6,IL-6)、白细胞介素-10(interleukin-10,IL-10)含量及相关临床指标的变化,为牙周炎的临床诊断和治疗提供理论依据。方法:采集40例健康人的40个健康牙位及轻度与重度牙周炎组各40个炎症牙位的GCF,采用酶联免疫吸附分析(ELISA)检测治疗前及治疗后6wk、8wk、10wkGCF中IL-6及IL-10的含量并分别记录治疗前、后的探诊深度(probing depth,PD)、附着丧失(attachment loss,AL)、菌斑指数(plaque index,PLI)和龈沟出血指数(sulcus bleeding index,SBI),并分析其相关性。结果:(1)牙周炎组病人治疗前、后GCF中IL-6含量均显著高于健康对照组(P<0.01),重度牙周炎组病人治疗前、后GCF中IL-6含量均高于轻度牙周炎组(P<0.01),且牙周治疗后牙周炎组GCF中IL-6含量明显降低,并且牙周炎病人GCF中IL-6含量与PD、AL、PLI和SBI分别呈正相关关系(P<0.05)。(2)牙周炎组病人GCF中IL-10含量显著低于健康对照组(P<0.01),重度牙周炎组病人GCF中IL-10含量低于轻度牙周炎病人(P<0.01),且牙周治疗后牙周炎组GCF中IL-10含量明显升高,牙周炎病人GCF中IL-10含量与PD、AL、PLI和SBI呈负相关关系(P<0.05)。(3)IL-6与IL-10在相应时间点呈负相关关系(P<0.05)。结论:IL-6含量在牙周炎基础治疗后10wk内是逐渐降低的,而IL-10含量在治疗后10wk内是逐渐升高的,两因子在相应的时间点呈负相关关系。Objective：To explore the changes of clinical markers and content in the Interleukin-6（IL-6）,interleukin-10（IL-10）levels before and after the periodontal treatment 6,8,10wk of gingival crevicular fluid（GCF）with periodontitis.Methods：Collected gingival crevicular fluid from 40 healthy teeth with healthy people;Mild and severe periodontitis groups of 40 teeth of the GCF inflammatory.Using enzyme-linked immunosorbent assay（ELISA）to detect the IL-6 and IL-10 levelsin GCF,and record the probing depth（PD）,attachment loss（CAL）,plaque index（PLI）and gingival sulcus bleeding index（SBI）,before and after treatment 6,8,10wk,analyzing their relevance.Results：Before and after treatment,IL-6 levels in GCF of periodontitis patients were significantly higher than healthy controls（P〈0.01）;IL-6 levels in GCF of severe periodontitis were higher than those with mild periodontitis group（P〈0.01）.After periodontal therapy,the IL-6 of periodontitis group were significantly reduced.PD,AL,PLI and SBI were positive correlation（P〈0.05）,while IL-10 of GCF in patients with periodontitis was significantly lower than healthy controls（P〈0.01）,IL-10 of GCFin patients with severe periodontitis was lower than in patients with mild periodontitis（P〈0.01）.After periodontal therapy,the IL-10 of periodontitis group were significantly increased.PD,AL,PLI and SBI was negatively correlated（P〈0.05）,IL-6 and IL-10 are in the corresponding negative correlation between time points（P〈0.05）.Conclusion： IL-6 levels after treatment is a gradual decrease in 10wk,while the IL-10 levels is gradually increased in 10wk,in the corresponding time point the two factors IL-6 and IL-10 are negative correlation.
