CD44与p-糖蛋白在食管鳞状上皮细胞癌中表达的相关性及临床意义

摘　　要：目的:探讨CD44、p-糖蛋白(P-gp)的表达在食管癌中相关性及临床意义。方法:应用免疫组织化学法技术检测62例食管鳞状上皮细胞癌患者组织标本中CD44蛋白和P-gp的表达情况,根据检测结果进行统计学分析。结果:62例食管鳞状上皮细胞癌中CD44蛋白表达阳性39例(62.9%),P-gp表达阳性21例(33.9%);食管癌及癌旁正常上皮中CD44蛋白与P-gp表达的差异有统计学意义(P<0.001);CD44蛋白与P-gp表达存在相关性(P=0.035);CD44蛋白的表达与患者的性别年龄无关,与肿瘤的分化程度,浸润深度,淋巴结转移相关;P-gp表达与患者的性别,年龄,分化程度无关,与肿瘤的分期,淋巴结转移相关。在CD44蛋白和P-gp蛋白同时表达的17例患者,淋巴结转移率高达70.1%(12/17),而CD44蛋白和P-gp同时不表达的患者中,只有15.7%(3/19)的病人发生淋巴结转移,两组差别有统计学意义(P<0.001)。结论:CD44与P-gp的联合检测有助于综合判断食管癌的分化程度,转移潜能及临床分期,有望为断肿瘤预后的新指标;CD44与P-gp的表达存在相关性,它们对肿瘤多药耐药的影响值得进一步深入研究。Objective：The arm of the study was to evaluate relationship between the expression of CD44 protein and P-glycoprotein（P-gp） in tissue of esophageal squamous cell carcinoma（ESCC）,and to assess its clinical significance.Methods：The expression of CD44 protein and P-gp was detected in the tissue of ESCC at the case of 62 patients using the method of immunohistochemistry.Its result was calculated with the software of SPSS 11.5.Results：The positive rate of CD44 protein in the tumor tissue of squamous esophageal mucosa is 62.9 %（39/62）,alternatively,the rate of P-gp is 40.3%（25/62）;There exists correlation between the expression of（P=0.035）;The expression of CD44 protein is closely related to differentiation degree,tumor stage,and lymphatic metastasis（LM）,but not to patient sex or age.Furthermore,the expression of P-gp are associated with tumor stage,and metastasis of lymph node,but not to patient sex,age,and tumor differentiation degree;In the patients with positive Co-expression of CD44 protein and P-gp（17/62）,rate of LM is highly as 70.1%（12/17）;However,there are only 15.7%（3/19） patients occurred LM within the group of negative expression;The expression rate of CD44 and P-gp in the patients with LM is higher than the patients without LM（P0.001）.Conclusion：The joint detection of CD44 protein and P-gp in the cancer tissue of squamous esophageal mucosa may contribute to estimate tumor differentiation degree,stage,and metastatic potential,which may become a new marker for assessing prognosis of cancer patients;Moreover,the expression of CD44 are related to P-gp and further researches are encouraged in this field for treating metastasis and multidrug resistance of the tumor cells.
