术中超声在肝癌切除术中的临床应用价值

摘　　要：目的:探讨术中超声(IOUS)在肝癌切除手术中的临床应用。方法:术中超声引导下对56例肝癌患者行肝部分切除术或肝部分切除联合无水乙醇注射治疗,并将术前常规超声、增强CT及术中超声检查结果与病理诊断结果进行对比分析。结果:术前常规超声、术中超声、增强CT对比分析表明,在肝癌尤其微小肝癌和肝癌小子灶中,术中超声对病灶的显示率优于术前常规超声和增强CT,差异有统计学意义(P<0.05)。结论:术中超声图像清晰、定位准确、在确定肝切除量和手术的完整性上有明显的优势,有重要的临床应用价值。Objective To explore the clinical value of intraoperative ultrasound（OUS） in hepatectomy for liver cancer.Methods OUS-guided were performed in partial hepatectomy or partial liver resection combined with percutaneous ethanol injection therapy on 56 cases of liver cancer.The results of OUS were compared with preoperative ultrasonography and enhanced CT according to pathological diagnosis.Results OUS exceed preoperative ultrasonography and enhanced CT in detecting small hepatic cancer and metastases.The difference of detection rates was statistically significant（P 0.05）.Conclusion OUS image is clear and precisely guide.OUS has unique advantage in the integrity of the liver resection and surgery.The clinical value of OUS is significant in hepatectomy.
