彩色多普勒超声检测腔隙性脑梗死患者颅外段椎动脉血流的临床价值

摘　　要：目的探讨彩色多普勒超声对腔隙性脑梗死患者颅外段椎动脉血流的诊断价值。方法 56例腔隙性脑梗死患者和30例健康体检者为对照组,彩色多普勒超声检测其颅外段椎动脉的血管内径及血流参数。结果与对照组比较,腔隙性脑梗死患者颅外段椎动脉管壁增厚不光滑,血管内径减小,收缩期峰值流速减低,阻力指数增大,差异均有统计学意义(均P<0.05)。结论彩色多普勒超声可以客观评定颅外段椎动脉结构及血流状况,为伴有椎-基底动脉供血不足的腔隙性脑梗死患者的诊断提供依据。Objective To study the value of color Doppler uhrasonography（CDU） for the examination of the extracranial vertebral artery flow in patients with lacunar infarction. Methods Both sides of extracranial vertebral arteries in 56 patients with lacunar infarction and 30 healthy controls were measured by CDU instrument. Results The vertebral arteries of the patients showed the thickened vessel wall, decreased vessel diameters and low PSV as well as high resistance index. There was significant difference was found between the lacunar infarction group and the control（P〈0.05 ）. Conclusion The structure and blood apply of vertebral arteries can be objectively evaluated with CDU. It can provide the basis for the diagnosis of lacunar infarction with vertebrobasilar ischemia.
