摘  要:目的通过建立颅脑爆震伤动物模型,探讨颅脑爆震伤后血浆中肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)的变化,比较不同伤情与血浆TNF-α、IL-6含量的关系。方法杂种雄性犬30只,随机分为:轻伤I组(13 mm,n=10),中型伤II组(9 mm,n=10)及重伤III组(5 mm,n=10),用1 g TNT当量的球型爆炸源置于距右颞顶部不同距离(13mm、9mm、5mm)对犬的颅脑部引爆致伤,制作轻、中、重型颅脑爆震伤动物模型,伤前及伤后0 h、1 h、2 h、4 h、6 h、12 h对血浆中TNF-α、IL-6指标监测。结果犬爆震伤1 h后,中、重伤组血浆中TNF-α、IL-6含量与伤前比较明显升高(P<0.01),在不同时间点重伤组与轻伤、中型伤组对比有明显差异(P<0.05);中型伤组TNF-α、IL-6较轻型伤组略高,但无明显差异(P>0.05)。双变量相关统计分析表明,在伤后6 h时血清中TNF-α、IL-6含量与颅脑损伤程度呈正相关。结论颅脑爆震伤后血浆中TNF-α、IL-6升高,其升高程度与伤情轻重程度紧密相关,提示TNF-α、IL-6可能参与了早期的颅脑损伤过程。Objective To establish the animal models with craniocerebral explosive injury in dogs and observe the changes of tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) contents in the serum follwing craniocerebral explosive injury.Methods Thirty mongrel male dogs were randomly divided into slight injury I group(13 mm),moderate injury II group(9 mm) and severe injury III group(5 mm),with 10 in each group.A spherical explosive with explosive energy of 1 g TNT was used to detonate at different distances(13 mm,9 mm,5 mm) to dogs' heads to establish the slight,moderate and severe craniocerebral explosive injury animal models,respectively.The contents of TNF-α and IL-6 in the serum were observed in every group before explosion and at 0 h,1 h,2 h,4 h,6 h and 12 h after explosion.Results The contents of TNF-α and IL-6 in the serum of dogs suffered moderate injury or severe injury at 1 h after explosion were significantly increased than that before injury.There was significant difference in the contents of TNF-α and IL-6 at different time points among slight,moderate injury group and severe injury group(P0.05);the contents of TNF-α and IL-6 in the moderate injury groups were slightly higher than those of the slight injury groups,but no significant difference was observed(P0.05).Analysis of relevant factors indicated that the contents of TNF-α and IL-6 were in positive correlation with the severity of craniocerebral explosive injury.Conclusion The contents of TNF-α and IL-6 in the serum of dogs are increased at early stage after craniocerebral explosive injury,which is closely related with the severity of explosive injury in dogs.It is suggested that TNF-α and IL-6 may play a role at the early stage of traumatic brain injury.
