超声生物显微镜在经巩膜睫状体光凝治疗晚期难治性青光眼中的应用

摘  要:目的探讨不同能量半导体激光经巩膜睫状体光凝术(transscleral diode laser cyclophotocoagulation for the late refractory glaucoma,TSCPC)治疗晚期难治性青光眼的临床疗效。方法将2006年10月至2008年11月在天津医科大学眼科中心诊治的晚期难治性青光眼患者59例62眼随机分为两组,分别应用高能量和低能量行TSCPC。观察手术前后眼压,并应用超声生物显微镜(ultrasound biommicroscopy,UBM)观察TSCPC治疗前后睫状体厚度变化。结果高能量组眼压由术前(45.65±11.31)mmHg(1kPa=7.5mmHg)降至术后(19.27±7.27)mmHg,低能量组眼压由术前(44.18±10.89)mmHg降至(27.53±9.47)mmHg,两组术后眼压差异有显著统计学意义(P<0.01)。应用UBM进行测量,高能量组术前和术后1d、1周、1个月、3个月睫状体厚度分别为(1.19±0.20)mm、(1.34±0.18)mm、(1.30±0.25)mm、(1.21±0.20)mm、(1.01±0.28)mm;低能量组术前和术后1d、1周、1个月、3个月睫状体厚度分别为(1.20±0.23)mm、(1.23±0.22)mm、(1.22±0.29)mm、(1.21±0.19)mm、(1.15±0.24)mm。两组睫状体厚度术前比较差异无统计学意义(t=0.28,P>0.05)。高能量组术前和术后1d、1周、3个月比较差异有显著统计学意义(F=10.09,P<0.01),术前和术后1个月比较差异无统计学意义(P>0.05)。而低能量组术前和术后1d、1周、1个月、3个月比较差异无统计学意义(F=0.52,P>0.05)。结论 TSCPC治疗晚期难治性青光眼能够有效降低眼压,不同能量治疗的疗效及睫状体的形态反应不同。Objective To evaluate the clinical efficacy of transscleral diode laser cyclophotocoagulation with different power on late refractory glaucoma. Methods A total of 59 patients (62 eyes) with refractory glaucoma in Eye Center of Tianjin Medical University from October 2006 to November 2008 were chosen and randomly divided into two groups, the diode laser cyclophotocoagulation with high and low power were performed, respectively. The intraoculax pressure (IOP) before and after operation was observed, and the ciliary body thickness was measured by ultrasound biommlcroscopy (UBM). Results The preoperative and postoperative IOP in higher power group were (45.65 ± 11.31 ) mmHg ( 1 kPa = 7.5 mmHg) and ( 19.27 ± 7.27 ) mmHg, respectively, which in low power group were (44. 18 ± 10. 89 ) mmHg and ( 27.53 ± 9.47 ) nmHg, there was significant difference in postoperative IOP between two groups ( P 〈 0.01 ).
