天津市蓟县桑梓村40岁及以上人群原发性闭角型青光眼相关危险因素研究

摘　　要：目的探讨原发性闭角型青光眼的相关危险因素。方法对2003年1。3月在天津蓟县桑梓村进行青光眼的普查，并通过问卷调查获取资料，对≥40岁人群进行了原发性闭角型青光眼的相关因素分析。结果≥40岁人群共1987人，1701人接受了检查及问卷调查，应答率为85．61％。单因素分析显示年龄（OR=2．186，95％CI：1．409～3．394）、青光眼家族史（OR=6．009，95％CI：2．151～17．253）、眼痛眼胀（OR=4．485，95％CI：1．590～12．665）、一过性视物模糊或虹视（OR=5．651，95％CI：2．112～15．119）及白内障（OR=3．318，95％CI：1．290～8．481）与原发性闭角型青光眼呈正相关，文化程度（OR=0．306，95％CI：0．122—0．765）与原发性闭角型青光眼呈负相关。多因素分析显示年龄（OR=2．146，95％CI：1．239～3．719）及青光眼家族史（OR=6．604，95％CI：2．141～20．374）均为原发性闭角型青光眼的独立危险因素。结论在青光眼的筛检、普查中，应重视青光眼症状的询问，对于高龄伴有青光眼家族史者应定期随访。Objective To evaluate several risk factors of primary angleclosure glaucoma.Meth- ods All 1987 subjects residing in Sangzi village,Ji County were enrolled to undergo an ophthalmo- logic examination and questionnaire from January to March in 2003.By using questionnaire, some in- formation were obtained to analysis the risk factors. Result In the survey, 1701 individuals were enumerated, the response rate was 85.61%.The risk for developing primary angle closure glaucoma was increasing with age（OR=2.186,95%CI： 1.409-3.394）.family history of glaucoma（OR=6.009,95%CI： 2.151-17.253）, the symptom of ophthalmalgia or distending pain （OR=4.485,95 % CI： 1.590-12.665）, eva- nescent temporary eye blurring or iridization （OR=5.651, 95%CI： 2.112--15.119）and cataract （ORTM 3.318,95%CI： 1.290-8.481）,but decreasing with degree of education （OR=0.306,95%CI：0.122-0.765）. Age（OR=2.146, 95%CI： 1.239-3.719）and family history of glaucoma（OR=6.604, 95%CI：2.141-20.374） were independent risk factors for primary angle closure glancoma.Conclusion During the census of glaucoma, the symptom is important too.Individuals who are old and have history of glaucoma should be received ophthalmologic examinations.
