暴发性脉络膜上腔出血对视觉细胞结构和功能的影响

摘　　要：目的 探讨器发性脉络膜上腔出血（SHE）后的电生理、生化代谢及电镜下超微结构的变化特征，方法制备暴发性脉络膜出血之动物模型，选择不同时期的闪光视觉诱发电位（FVEP）、超氧化物歧化酶（SOD）及视网膜超微结构改变，明确其各自对视功能的损害。结果 ①FVEP：不同观察时期均有不同程度的潜伏期延长和振幅降低，统计学处理差异有显著性（P〈0．01）。②SOD值不同观察时期均有不同程度的下降，统计学处理有显著性差异（P〈0．01）。⑧电镜下细胞间隙增大，排列稀疏紊乱，胞体收缩变形，胞质自溶出现空泡，核崩解、固缩、浓染，线粒体嵴减少或消失，视细胞外节连接疏松。结论①FVKP的变化，从电生理学角度反映了SEH对网膜和视神经的损伤。②SOD值的下降，从生化代谢的角度反映了网膜组织氧化与抗氧化失衡，造成网膜的不可逆损伤。电镜下超微结构的改变，进一步证实了SEH是一种严重危害视功能的并发症。Objective To investigate the feature of supraehoruidal expulsive hemorrhage on the tlash visual evoked potenlial,biochemical melabolisnl and the eleetrun microscopic cell structure. Methods The animal model of supraehoroidal expulsive hemorrhage was made sueessfully,the flash visual evoked potential,the superoxide dismutase and the electron micruscopic cell structure were analyzed at dif- ferent time period,the damages of retina were studied. Results （1）FVEP PI wave latency was longer and amplitude was lower than normal control group,there were statistieally signifieant. （2）SOD coneentralion in retinal tissue was significantly lower than normal eonlrol group. Under the electron microscope observation, the spaces among cells broadened, cells arranged disot＇derly and sparsely, eell body were contracted amt deformated,eyluplasm autolysis, vacunlalion and pyknosis uverstain appeared, cell nucleus disinteglnted, and mitochomh＇ial cris/a decreased or disappeared s the rod outer segment connected loosely. Conclusion （I）Flash visual evoked potential expresses the damage of retina and optic nerve from SEH about the eleetrophysiolo change. （2）Redueed SOD concentration representes the variatiou of biochemical metabo- lism from SEll. （3）The ultrastruelure pathology changes suggesles that supraehoroidal expulsive hemorrhage is a serious complication badly to the vision.
