435例老年胃癌根治术后临床病理及预后分析

摘　　要：目的:分析老年胃癌患者预后因素及不同年龄组老年胃癌患者的临床病理特点及其预后差异。方法:回顾性分析天津医科大学附属肿瘤医院1996年7月至2006年7月间行胃癌根治术的435例老年胃癌患者(年龄≥65岁)的临床病理特点及其预后影响因素结果:单因素分析结果显示年龄分组、肿瘤部位、手术方式、联合切除、病理类型、肿瘤大小、T分期、N分期、临床分期是影响老年胃癌患者预后的因素。多因素分析结果显示年龄分组、联合切除、T分期、N分期是影响老年胃癌患者预后的独立因素对比分析不同年龄组老年胃癌患者的临床病理特点,结果显示肿瘤部位、手术方式、肿瘤大小、术后化疗在不同年龄组老年胃癌患者间比较差异有统计学意义(P<0.05)。对不同年龄组老年胃癌患者的预后进行分析结果显示不同年龄组老年胃癌患者的预后影响因素不同结论:不同年龄组老年胃癌患者在临床病理特点、预后影响因素方面存在差异,将老年胃癌患者分为不同年龄组进行研究具有重要意义,可以更好的指导治疗和判断预后。Objective： To analyze the prognostic factors of gastric cancer in elderly patients with gastric cancers, as well as their clinicopathologic characteristcs and the differences among the prognosis of the patients in various age groups. Methods： The clinico- pathological characteristcs and prognostic factors of 435 elderly patients with gastric cancer （/〉 65 years）, who have undergone radical gastrectomy in the Tianjin Cancer Hospital from July 1996 to July 2006, were analyzed retrospectively. Results： The univariate analysis results revealed that age group, tumor location, type of gastrectomy, resection of the other organs, pathological type, tumor size, T stage, N stage, and clinical stage were associated with the prognosis of gastric cancer in elderly patients. The multivariate analysis re- sults revealed that age group, resection of other organs, and the T and N stage were the independent factors of the prognosis. Contras- tive analysis of the clinicopathological characteristics was conducted in the three groups. The result revealed that significant differences exist in the tumor location, type of gastrectomy, tumor size, and postoperative chemotherapy among different age groups （P〈0.05）. Moreover, the prognostic factors varied among various age groups. Conclusion： Significant differences were observed in the clinico- pathologic characteristics and prognostic factors among different age groups. Studies on elderly patients with gastric cancers in differ- ent age groups are of great importance in the prediction of the prognosis.
