摘  要:目的探讨磁敏感加权成像(SWI)在急性缺血性脑卒中伴脑内微出血(CMBs)的诊断价值。方法将57例急性缺血性脑卒中患者行T_1WI、T_2WI、T_2FLAIR及SWI检查。分析比较常规序列与SWI对CMBs的检出率,探讨CMBs发生部位的特点,CMBs与脑白质变性程度的相关性。结果共发现21例患者有CMBs,CMBs灶总数为179个,在各部位的分布按数目依次为丘脑基底节区、皮质及皮质下、脑干、小脑。CMBs分级与脑白质变性的级别程度之间呈显著正相关(P<0.05)。结论 SWI在诊断CMBs的检出例数及病灶数上都显著优于常规序列。CMBs多发部位与所患疾病导致受累血管相关。CMBs的发生与脑白质变性、小血管疾病密切相关。Objective To assess the diagnostic value of susceptibility-weighted imaging (SWI) for cerebral microbleed (CMB) in acute cerebral arterial thrombosis. Methods MRI was performed on 57 patients with acute cerebral arterial thrombosis. The detection rates of CMB were compared among T1WI, T2WI, TEFLAIR and SWI. The location of CMB and the association of CMB with myelinopathy were also evaluated. Results There were 21 patients with 179 CMB located more commonly in the basal ganglia than cortex-subcortex,brainstem and cerebellum. There was positive correlation between the degree of CMB and that of myelinopathy (P〈0.05). Conclusion SWI is more effective than conventional MRI sequences for depicting CMB. The incidence of CMB is closely correlated with myelinopathy and small vessel disease.
