奥沙利铂联合卡培他滨新辅助化学疗法治疗中晚期胃癌疗效观察

摘　　要：目的观察奥沙利铂联合卡培他滨新辅助化学疗法治疗中晚期胃癌的临床疗效。方法 72例中晚期胃癌患者分为2组,观察组33例给予奥沙利铂联合卡培他滨术前辅助化学治疗、手术治疗和术后化学治疗;对照组39例给予手术治疗加术后化学治疗,比较2组患者手术无肿瘤残留(R0)切除率、近期疗效及不良反应。结果观察组和对照组患者手术R0切除率分别为84.8%和59.0%,观察组R0切除率显著高于对照组(P<0.05);2组患者不良反应发生率比较差异无统计学意义(P>0.05);观察组和对照组治疗总有效率分别为54.5%和33.3%,观察组治疗总有效率显著高于对照组(P<0.05)。结论奥沙利铂联合卡培他滨新辅助化学治疗方案能提高中晚期胃癌患者的R0切除率和近期疗效。Objective To observe the effects of neoadjuvant chemotherapy of oxaliplatin combined with capecitabine in the treatment of advanced gastric cancer.Methods Seventy-two patients with advanced gastric cancer were divided into two groups,thirty-three patients in observation group were treated with neoadjuvant chemotherapy of oxaliplatin and capecitabine before operation,operation,and postoperative chemotherapy;while thirty-nine patients in control group were treated with operation and postoperative chemotherapy.The R0 resection rate,short-term effect and adverse reaction were compared between two groups.Results The R0 resection rate of the observation group and control group were 84.8% and 59.0% respectively,the R0 resection rate of the observation group was significantly higher than that of the control group（P0.05）.The total effective rate of the observation group and control group were 54.5% and 33.3% respectively,the total effective rate of the observation group was significantly higher than that of the control group（P〈0.05）.Conclusion Oxaliplatin combined with capecitabine in the neoadjuvant chemotherapy programs for patients with advanced gastric cancer can improve the R0 resection rate and short-term effect.
