经尿道绿激光汽化术同时治疗非肌层浸润性膀胱肿瘤合并良性前列腺增生的安全性观察

摘　　要：目的评价经尿道绿激光汽化术同时治疗非肌层浸润性膀胱肿瘤合并良性前列腺增生（BPH）的安全性。方法选择2004年5月至2010年10月收治的同时行经尿道膀胱肿瘤和前列腺绿激光汽化术治疗的患者27例（观察组），并选取同期单行经尿道膀胱肿瘤绿激光汽化术治疗的患者27例作为对照组。比较两组临床病理学特征、术后复发率、进展率以及膀胱颈和前列腺部尿道肿瘤复发率。结果术后观察组和对照组随访时间分别为（28．61±19．53）个月和（30．20±21．46）个月，两组术后复发率[18．5％（5，27）比25．9％（7，27）]、进展率[3．7％（1，27）比0]以及膀胱颈和前列腺部尿道肿瘤复发率（0比0）比较差异均无统计学意义（P〉0．05）。结论经尿道绿激光汽化术同时治疗非肌层浸润性膀胱肿瘤合并BPH，并未增加肿瘤在前列腺部尿道的复发率。Objective To evaluate the safety of simultaneous transurethral green laser vaporization therapy in benign prostatic hyperplasia （BPH） and nonmuscle-invasive bladder transitional cell carcinoma （NMIBT）. Methods The clinical data of 27 patients （observation group） who had undergone simultaneous transurethral green laser vaporization therapy in BPH and NMIBT between May 2004 and October 2010 were analyzed retrospectively. Meanwhile 27 patients（control group） only had undergone green laser vaporization therapy in NMIBT during the same period were selected. Clinicopathologic parameters, rate of recurrence and progression, rate of recurrence in the bladder neck and prostatic urethra were determined and compared. Results The time of foUow-up in observation group and control group were （28.61± 19.53） and （30.20 ± 21.46） months. The rates of recurrence, progression and recurrence in the bladder neck and prostatic urethra between observation group and control group had no significant differences [ 18.5% （5/27） vs. 25.9% （7/27）,3.7% （1/27） vs. 0, 0 vs. 0] （P 〉 0.05）. Conclusion Simultaneous transurethral green laser vaporization of NMIBT and BPH can be safely performed without increasing the risk of tumor recurrence in the prostatic urethra.
