摘  要:目的:探讨磁敏感加权成像在脊髓急性外伤性出血中的应用价值。方法:21例脊髓急性外伤出血的患者行3.0T磁敏感加权成像(SWI)、二维扰相梯度回波(T2*加权2D-MERGE T2*WI)及常规快速自旋回波T1加权FSE-T1WI、快速自旋回波T2加权FSE-T2WI序列扫描,病变组所有病例经临床随访及病理证实,包括颈段脊髓挫裂伤12例、胸段脊髓挫裂伤6例、胸腰段脊髓挫裂伤3例。比较分析SWI及T2*WI、T2WI对脊髓出血性病变检出的敏感性和特异性。11例正常志愿者作为对照组。结果:21例脊髓挫裂伤常规FSE-T2WI均显示水肿征象,其中6例在SWI、2D-MERGE T2*WI及FSE-T1WI、FSE-T2WI均未见出血征象;7例仅在SWI显示出血征象;8例在SWI显示低信号出血征象较2D-MERGE T2*WI及常规FSE-T2WI更敏感。结论:磁敏感加权成像检出脊髓急性外伤性出血比常规MRI更敏感,可作为脊髓出血性病变MRI检查重要的补充序列。Objective: To evaluate the role of the susceptibility weighted imaging(SWI) in detecting spinal microhemorrhage in acute injury.Methods: Of the 21 traumatic cases,12 were cervical spinal contusion,6 were thoracic spinal contusion,and 3 cases were thoraco-lumbar spinal contusion.SWI,2D-MERGE-T^*2WI,FSE-T1WI,and FSE-T2WI were performed on all patients on 3.0T scanner.Meanwhile,11 volunteers were also scanned as a control group.The sensitivity and specificity in detecting spinal hemorrhage were compared.Results: FSE-T2WI showed the edema of all the 21 patients.Out of 21 patients,6 patients didn't show spinal hemorrhage on SWI,2D-MERGE-T^*2WI,and FSE-T2WI.Seven patients showed spinal hemorrhage on SWI only.SWI is more sensitive than 2D-MERGE-T^*2WI and FSE-T2WI in detecting bleeding in 8 patients.Conclusion: SWI is more sensitive than conventional MRI in detecting traumatic spinal microhemorrhage.This technique is an important tool in the routine evaluation of the spinal hemorrhagic diseases.
