摘  要:目的:研究宫颈组织中朗格汉斯细胞(LC)的数量、形态及分布特点与宫颈癌及人乳头瘤病毒(HPV)16E6的关系。方法:用免疫组织化学技术(SP染色法)对标本的朗格汉斯细胞进行检测,以直接PCR法检测HPV16E6并对E6基因测序。结果:40例宫颈鳞癌,16例宫颈上皮内瘤变(CIN)Ⅱ/Ⅲ级与14例正常宫颈组织中,S-100蛋白阳性LC数量在正常组、CINⅡ/Ⅲ组、宫颈癌组中依次增加(P=0.000),与宫颈癌的临床分期呈负相关(R=-0.552,P=0.000),随宫颈癌组织分化程度降低而减少(P=0.039),与淋巴结转移无关(P=0.245)。HPV16E6在正常对照组、CINⅡ/Ⅲ组和浸润癌组中的阳性率依次上调(P=0.000)。HPV16E6(+)组中LC数量比HPV16E6(-)组少(P=0.011)。LC的表达在HPV16E6原型组和在HPV16E6D25E突变株组中无统计学差异(P=0.813)。结论:LC功能缺陷与宫颈癌的发生有关。Objective: To examine the relationship between Langerhans' cells(LCs) and cervical squamous-cell carcinoma(SCC),and to study the effect of HPV16 E6 on the changes of LCs in normal cervix epithelium,cervical intraepithelia neoplasia(CIN),and SCC.Methods: The identification of LC was done by immunohistochemical staining on S-100 protein.HPV16 E6 was determined by polymerase chain reaction(PCR) and PCR-directed sequencing.Results: The number of LCs(S-100 positive) was higher in cervical cancer than in normal cervix epithelium(P=0.000).S-100 protein expression was negatively correlated to the clinical staging of SCC(r=-0.552,P=0.000),and SCC differentiation(P=0.039).There was no statistically significant difference,however,between lymphatic metastasis group and lymphatic without metastasis(P=0.245).The positive rate of HPV16 E6 was 14.3%,56.25%,and 82.5% in normal group,CINⅡ/Ⅲ group and SCC group respectively(P=0.000).S-100(+) LCs in HPV16 E6(+) cases were significant less than in HPV16 E6(-) cases(P=0.011).No significance difference was found between the number of S-100 LCs in D25E variation group and prototype sequences group(P=0.813).Conclusion: The defective function of LC was closely correlated to cervical cancer.
