早期乳腺癌保乳术后化疗加三维适形放疗的临床疗效分析

摘　　要：目的研究保乳术后化疗加三维适形放疗治疗I～II期乳腺癌的临床疗效。方法将65例I—II期女性乳腺癌患者分为保乳术组31例和改良根治术组34例。术后均辅以化疗和三维适形放疗。对ER、PR阳性的患者给予内分泌治疗。结果保乳术纽和改良根治术纽放射不良反应发生率、并发症发生率、局部复发率、远处转移率及5年生存率差异均无统计学意义（P〉0．05），但保乳术组乳房外观美容效果和患者生存质量则明显优于改良根治术组（P〈0．05）。结论I～II期乳腺癌保乳术后化疗加三维适形放疗生存率高，复发率和转移率低，不良反应少，尤其是极大提高了美容效果和生存质量，在严格把握适应证和遵循治疗原则的前提下可作为治疗I～II期乳腺癌的首选方案。Objective To study the clinical efficacy of chemotherapy combined with three dimensional conformal radiotherapy after breast-conserving surgery for stage I and II breast cancer. Meth- ods Sixty-five patients with stage I or II breast cancer were divided into breast-conserving group （31 cases） and modified radical mastectomy group （34 cases）. The patients in the two groups all re- ceived conventional postoperative chemotherapy and three-dimensional conformal radiotherapy. Patients with positive estrone receptor or progesterone receptor were given endocrine treatment. Results There was no significant difference in radiation toxic reaction rate, complication rate, local recurrence rate, dis- tant metastasis rate or 5-year survival rate between the two groups （P 〉 0. 05 ）, but the breast-conserving group obtained better cosmetic results and survival quality than the modified radical mastectomy group （P 〈 0.05 ）. Conclusions Chemotherapy combined with three dimensional conformal radiotherapy after breast-conserving surgery can be taken as the preferred scheme for stage I and II breast cancer as long as the indication and the principal are strictly obeyed, because it has high survival rate, low local recurrence rate, low distant metastasis rate and side-effect rate, especially improve the cosmetic result and survival quality.
