包套式食管全层胃黏膜吻合术在食管癌切除术中的应用

摘　　要：目的探讨包套式食管全层胃黏膜吻合术在食管癌切除术中吻合口瘘、吻合口狭窄和反流性食管炎的临床应用。方法将412例食管癌切除术患者随机分为两组，观察组235例采用包套式食管全层胃黏膜吻合术，对照组177例采用食管胃分层吻合术，比较两组患者术后吻合口瘘、吻合口狭窄和反流性食管炎的发生情况。结果观察组和对照组均进行了随访，观察组发生吻合口瘘0例，对照组1例。观察组发生吻合狭窄2例，对照组8例。观察组发生反流性食管炎8例，对照组31例。观察组发生吻合口瘘、吻合口狭窄和反流性食管炎明显低于对照组（P〈0．05）。结论包套式食管全层胃黏膜吻合术可以降低食管癌切除术后吻合口瘘、吻合口狭窄和反流性食管炎的发生率，提高生存质量。Objective To investigate the sheath-style full-thickness esophageal anastomosis used in esophageal mucosal resection of anastomotic leakage, anastomotic stenosis and reflux esophagitis. Methods Four hundred and twelve cases of esophageal cancerresection were divided into two groups, 235 cases in observation group recieved the implementation of sheath-style full-thickness esophageal mucosa anastomosis in the control group, 177 cases in the in control group were given layered esophagogas- tric anastomosis. The postoperative anastomotic fistula, anastomotic stenosis andreflux esophagitis were observed. Results The observation group and control group were followed up for 1 year after treatment, no one occurred anastomotic leakage in observation the group, and 1 case oeeured anastomotic leakage in the control group, 2 patients anastomotie stenosis in observation group, and 8 cases in the controlgroup, 8 cases occurred reflux esophagitis in observation group, and 31 cases in control group. The difference was significant （P 〈 0. 05 ）. The incidences of anastomotic leakage, anastomotic stenosis and reflux esophagitis were significantly lower in observation group than those in the control group. Conclusions Sheath-style full-thickness esophageal mucosaanastomosis after resection of esophageal cancer can reduce anastomotic leakage, anastomotic stenosis and reflux esophagitis incidence and improve the quality of life
