NSE、Cyfra21-1和CEA在小细胞肺癌与非小细胞肺癌检测中的敏感性比较

摘　　要：目的通过检测比较小细胞肺癌与非小细胞肺癌血清中癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(Cyfra21-1)的水平,探讨三个指标在小细胞肺癌(SCLC)和非小细胞肺癌(NSCLC)检测中的敏感性。方法共检测小细胞肺癌患者43例,非小细胞肺癌78例,健康对照组60例,测定各组血清CEA、NSE、Cyfra21-1水平,比较各组差异。结果非小细胞肺癌组和小细胞肺癌组CEA、NSE、Cyfra21-1浓度明显高于正常对照组(P<0.05),NSE在小细胞肺癌患者中的阳性检出率明显高于非小细胞肺癌组,且具有统计学意义(P<0.05),Cyfra21-1在非小细胞肺癌组的阳性检出率明显高于小细胞肺癌组,且具有统计学意义(P<0.05),CEA在两组间阳性检出率差异无统计学意义(P>0.05)。结论 NSE对小细胞肺癌较为敏感,Cyfra21-1对非小细胞肺癌较为敏感,NSE和Cyfra21-1在小细胞肺癌与非小细胞肺癌鉴别诊断中具有重要辅助意义。ObjectiveTo investigate the sensitivity of CEA, NSE and Cyfra21 -1 in SCLC and NSCLC by comparison with the level of serum CEA, NSE, Cyfra21 - 1 in the two groups. Methods Serum CEA, NSE and Cyfra21 - 1 were detected in 43 paitents of SCLC and 78 patients of NSCLC and 60 patients as healthy control group, then compared the difference of three groups. Results The level of Serum CEA,NSE and Cyfra21 - 1 in SCLC and NSCLC were differently higher than the healthy control group （ P 〈 0.05 ） , the positive rate of NSE in SCLC differently higher than NSCLC, the differ- ence had statistical significance （P 〈0. 05 ）, the positive rate of Cyfra21 - 1 in NSCLC differently higher than that of SCLC, the difference had statistical significance （P 〈 0. 05 ）, the positive rate of CEA has no statistical significance between the two groups （P 〉 0. 05 ）. Conclusions NSE is sensitive in SCLC, Cyfra21 -1 is sensitive in NSCLC, NSE and Cyfra21 - 1 have important auxiliary value in differential diagnosis of SCLC and NSCLC.
