摘　　要：目的探讨综合介入治疗复杂性巨大肝血管瘤的疗效及安全性。方法采用改良Seldinger技术行肝动脉插管至血管瘤供血动脉,对7例复杂性巨大肝海绵状血管瘤先注入少量平阳霉素碘油乳剂,再注入单纯碘油及明胶海绵颗粒栓塞;对2例复杂性巨大肝海绵状血管瘤用单纯碘油加明胶海绵微粒进行栓塞。在CT引导下对9例复杂性巨大肝海绵状血管瘤碘油缺失区再分次交替行经皮瘤体内直接注射平阳霉素碘油乳剂和无水乙醇。定期观察瘤体大小和并发症。结果综合应用肝动脉栓塞和经皮瘤体内直接注射治疗复杂性巨大肝血管瘤后,血管瘤体积显著缩小,无明显并发症。结论综合介入治疗复杂性巨大肝血管瘤具有疗效好、安全性高等优点,是较理想的治疗方法。Objective It is to explore the clinical curative effect and safety of comprehensive interventional therapy for complexity giant liver hemangioma. Methods Nine patients with complexity huge hepatic cavernous hemangioma were chosen, in which seven cases were treated by transcatheter superselective arterial PLE and lipiodol infusion with improved Seldingers＂ technology, and the supplying artery of the lesions was embolized by gelfoam particles （GF） , the other two cases were treated only with lipiodol and GF. Under CT guided, PLE and alcohol were injected to the unfilled area of the lesions at the same time. The size and iodized oil deposited situation of CHL were followed up after the embolization. Therapeutic effect and compli- cations were observed. Results All of the tumors decreased in size significantly after this comprehensive interventional therapy. No serious complication happened. Conclusion Comprehensive interventional therapy is an effective and safe method for treating the complexity huge hepatic cavernous hemangioma
