摘 要:目的观察人类主要细胞周期素[(Cyclins)D1、E、A、B1]在不同结直肠组织中的表达,探讨其在结直肠肿瘤形成、发展过程中的表达规律。方法采用流式细胞术(FCM)双参数法半定量检测30例正常结肠黏膜组织,28例结直肠腺瘤及121例结直肠癌组织中Cyclin(D1、E、A、B1)及细胞核增殖抗原(Ki一67)表达。根据不同Cyclin在细胞周期中的作用及表达特征,将其表达分为不同类型,结合临床病理学资料进行统计学分析。结果结肠黏膜底部、中间层、表层组织细胞,Cyclin(E、B1)表达阳性率及水平由高到低直至无表达,差异有统计学意义(P〈0.01);Cyclin(E、B1)表达与细胞增殖活性(以Ki-67为指标)正相关(P〈0.05);Cyclin(Dl、A)仅表达于底层增殖细胞,表层完全分化细胞不表达Cyclins。结直肠腺瘤组织细胞Cyclins的表达主要分3型:I型Cyclin(E、B1)低表达,Cyclin(D1、A)无表达。Ⅱ型Cyclin(E、B1)高表达,Cyclin(Dl,A)低/无表达;HI型Cyclin(D1,E,A,B1)高表达;Cyclins表达形式与结直肠腺瘤大小、病理类型及腺瘤增殖活性明显相关(P〈0.05)。结直癌细胞Cyclins异常表达大致可归为5型:I型Cyclin(D1、E、A、B1)均高表达;Ⅱ型CyclinD1低表达,Cyclin(E、A、B1)均不同形式高表达;III型CyclinDI不表达,Cyclin(E、A、B1)均不同形式高表达;1V型CyclinD1不表达,Cyclin(E、A、B1)低表达;V型Cyclin(D1、E、A、B1)均不表达。I到V型结、直肠癌淋巴结转移率、转移程度、肿瘤浸润程度及TNM分期Ⅳ期呈上升趋势,Cyclins表达类型与结、直肠癌以上临床病理因素相关(P〈0.05)。结论正常结肠黏膜细胞增殖分化过程中Cyclins(以CyclinE、B1为主)呈衰减规律性变化,可能与细胞有限增殖和定向分化的调控有关。Cyclins表达水平逐步增高可能与结直肠腺瘤的形成Objective To investigate the hallmark of primary Cyclins expressed in different color- ectal tissues, and illustrate the laws and significance of primary Cyclins expressed in progression of human eolorectal carcinogenesis. Methods Primary Cyelins ( D1, E, A, B1 ) and ki67 expressed in single cells detached from 30 cases of normal colon mueosa, 26 cases of eoloreetal adenomas and 121 cases of colorec- tal adenocareinomas were analyzed by using flow cytometry. The expression types of primary Cyelins were classified according to the roles and the patterns of primary Cyelins expressed in cell cycle. The results and clinieopathomorphological data were analyzed. Results The positive expression rate and the expression lev- els of primary Cyclins, especially Cyclins (E, B1 ) , were discrepant in different layers of normal colon mu- cosal tissues, which were attenuated gradually from crypt to luminal surface. The positive expression rate and the levels of Cyclins expressed were correlated with the activity of proliferation in colon mucosal tissue. The expression of Cyclins (D1, A) was only located in crypt cells. There were no Cyclins expressed in the differentiated cells, such as luminal surface cells. The expression patterns of primay Cyclins could be clas- sified three types in colorectal adenomas: Type I , low expression of Cyclins (E , B1 ), and no expression of Cyelins ( D1, A) ; Type 11, high expression of Cyelins ( E, B1 ) by different patterns, and low expres-sion of Cyclins (D1, A); Type m, high expression of Cyclins (D1, E, A, B1 ). The expression patterns of Cyclins were dift^rent in coloreetal adenomas with different size, pathological features, and proliierative activity. The expression patterns of primary Cyclins could be classified five types in colorectal adenocarci- nomas: Type 1 , high expression of Cyelins (1)1 , E, A, B1) ; Type lI , low expression of Cyclin D1, and high expression of Cyclins ( E, A, B1 ) by different patterns; Type m, no expression of Cyc
