乳腺浸润性导管癌与导管原位癌X线征象分析

摘　　要：目的 对比分析乳腺浸润性导管癌（invasive ductal carcinoma,IDC）与导管原位癌 （ductal carcinoma in situ,DCIS）的X线征象特点.方法 回顾分析经手术病理证实的172例乳腺导管癌的X线征象.结果 172例乳腺导管癌中,IDC 151例,DCIS 21例.（1）乳腺导管癌中单纯钙化、单纯肿块、肿块伴钙化及结构扭曲分别为40例（23%）、26例（15%）、51例（30%）、25例（15%）,其他征象共30例（17%）.IDC中肿块伴钙化为47例（31%）,DCIS中单纯钙化为13例（62%）.其中单纯钙化、单纯肿块及结构扭曲在两者之间有显著性差异（P=0.000,P=0.047,P=0.046）.（2）乳腺导管癌的肿块多为不规则形43例（56%）、分叶状17例（22%）,边界不清及伴毛刺均为34例（44%）.钙化多为多形性不均匀47例（43%）、不定形浅淡41例（38%）;分布以集簇状（32%）及区域性（27%）多见.结论 乳腺导管癌的X线征象丰富,IDC以肿块伴钙化最常见,而DCIS多表现为单纯钙化.Objective To analyze X-ray characteristics of invasive ductal carcinoma（IDC} and ductal carcinoma in situ（DCIS）. Methods 172 cases with breast duetal carcinoma confirmed by pathology were retrospectively analysed. Results Among 172 patients with breast ductal carcinoma, there were 151 cases with IDC and 21 cases with DCIS. The lesions appeared as pure calcifications in 40 of 172 （ 23%） , pure masses in 26 （ 15% ） , masses with calcifications in 51 （ 30 % ） , structural distortion in 25 （ 1,5% ） , and other signs in 30（17%）.In 151 patients with IDC,the lesions were shown as a mass with calcifications in 47（31%）,while in 21 patients with I）CIS, the lesions were showed as pure calcifications in 13 cases（ 62 % ）. The P values in the signs of pure calcification,pure mass and structural distortion were statistically significant between IDC and I）CIS（P= 0. 000, P：0. 047,P= 0. 046, respectively）. The masses in breast ductal carcinoma were irregular in 43（56 %},lobular in 17（22 %）,both ill-defined and spieulated in 34（44%）. The calcifications in the tumors were amorphous in 41 cases（38% } and pleomorphic in 47（43%）,and appeared as clustered and regional in 35（32% } and 29（27% ） ,respectively. Conclusion The mammographic features of the breast ductal carcinoma are variable, and the main mammographic feature of IDC is a mass with calcification,whereas DCIS usually appears as pure calcification.
