摘 要:目的 探讨动脉化疗栓塞对膀胱癌水通道蛋白1(aquaporin 1,AQP1)表达和细胞凋亡的影响.方法 选择膀胱癌患者30例,临床分期Tis 1例,Ta 2例,T1 6例,T2 11例,T3 9例,T4 1例;病理分级 G1 9例,G2 13例,G3 9例.经膀胱动脉缓慢灌注丝裂霉素20 mg或羟基喜树碱10 mg联合顺铂60 mg及明胶海绵颗粒栓塞.化疗栓塞后5 d经尿道膀胱肿瘤电切或膀胱部分切除术.动脉化疗栓塞前经膀胱镜下取得的肿瘤组织标本和术后取得的标本为实验组.30例正常膀胱组织黏膜(手术标本正常切缘组织)为对照组.2组标本应用免疫组化检测AQP1的表达和细胞凋亡测定.结果 AQP1在正常膀胱组织中主要表达于膀胱黏膜下微血管和小动脉内皮细胞;AQP1在膀胱癌动脉化疗栓塞前后肿瘤组织都有表达,但在动脉化疗栓塞后表达明显降低(t=31.43,P〈0.05).结论 动脉化疗栓塞可降低膀胱癌AQP1的表达,并使肿瘤细胞凋亡明显,能抑制肿瘤生长、转移,为一种有效的治疗方法.Objective To study the effect of arterial chemotherapy and embolization treatment on the expression of AQP1(aquaporin 1) and cell apoptosis in bladder cancer. Methods Thirty cases with bladder cancer ( Tis = 1 ,Ta = 2,T1= 6, T2=11, T3 = 9, T4 = 1; G1=9,G2= 13,G3=9) were treated by transcatheter arterial chemotherapy with Mitomycin(20 mg) or Hydroxycamptothecin {10 mg} adding cisplatin(60 mg} and embolization with gelatine sponge partclegelform 5 days before surgery. 30 cases with bladder cancers undergone arterial chemotherapy and embolization before tumor tissue specimens obtained by cystoscopy and postoperative obtained tumor specimens were considered as the experimental group. 30 cases with normal bladder tissue mucosa (operational specimens of normal margin tissue ) were considered as the control group. All specimens in Iwo groups were deah with immunohlstochemical method to detect the AQP1 expression and cell apoptosis. Results AQP1 in normal bladder tissues was mainly expressed in bladder submucosal microvessels and arteriolar endothelium. AQP1 in bladder cancer before and after transcatheter arterial chemo- therapy and embolization(TACE} was expressed in tumor tissue, But before and after the arterial chemotherapy and embolization, the expressed level of AQP1 remarkably decreased (t = 31.43, P〈0.001). The density of apoptosis of tumor tissue before the arterial chemotherapy and embolization was (31.49±5.59) / grid, after the arterial embollzation was ( 76.79± 17. 15) / grid ,and of normal bladder tissue was ( 8.00±3.29) / grid. The density of apoptosis of tumor cells after TACE was significantly higher than that of before (t=19.76,P〈0.001 ). There were no significant differences in the density of apoptosis of tumor cell between different pathologic staging and classification of the tumor cell apoptosis (P〉0. 05 ). Conclusion TACE is an effective therapeutic method for bladder cancer.
