摘 要:目的探讨胸水miRNA-21和miRNA-155含量检测的临床意义。方法分别从43例恶性胸水患者和27例良性胸水患者胸水中提取miRNA,采用荧光定量PCR技术检测miRNA-21和miRNA-155在良性和恶性胸水中表达量,常规细胞学方法检测胸水脱落细胞。应用受试者工作特征曲线(ROC)分析方法评价其诊断价值。结果恶性胸水组miRNA-21和miRNA-155的表达量明显高于良性胸水组(P<0.05),胸水脱落细胞学检查肿瘤细胞阳性组miRNA-21和miRNA-155的表达量明显高于阴性组。胸水中miRNA-21表达量对肺癌的诊断价值:当特异性为100%时,诊断的敏感性为67.4%,ROC曲线下面积0.891。胸水中miRNA-155表达量对肺癌的诊断价值:当特异性为100%时,诊断的敏感性为65.1%,ROC曲线下面积0.848。结论检测胸水miRNA-21和miRNA-155的表达对诊断恶性胸水具有较高的特异性和敏感性。Objective To investigate the diagnostic value of miRNA-21 and miRNA-155 expressions in pleural effusion.Methods The expressions of miRNA-21 and miRNA-155 were measured by RT-PCR in 43 patients with manlignant pleural effusion(group M) and 27 patients with benign pleural effusion(group B).The sediment of pleural effusion was examined by cytology.The microRNA expression in two groups was analyzed by receiver operating characteristic curve(ROC).Results The expressions of miRNA-21 and miRNA-155 in group M were much higher than those in group B,which were higher in group M with positive cancer cells than that without.ROC analysis of miRNA-21 in pleural effusion showed that when the specificity could reach 100%,the sensitivity for the diagnosis of manlignancy was 67.4% and the area under curve was 0.891.ROC analysis of miRNA-155 in pleural effusion showed that when the specificity could reach 100%,the sensitivity for the diagnosis of manlignancy was 65.1% and the area under curve was 0.848.Conclusion The measurement of miRNA-21 and miRNA-155 expressions in pleural effusion has higher sensitivity and specificity in the diagnosis of manlignant pleural effusion.
