益生菌对伪膜性肠炎TLR4表达的影响及意义

摘　　要：目的探讨益生菌对伪膜性肠炎患者TLR4表达的影响及意义。方法 22例伪膜性肠炎患者随机分成两组:常规治疗组和益生菌治疗组。观察两组的症状改善时间和副作用的发生率。收集伪膜性肠炎患者的外周血,提取单个核细胞,采用RT-PCR的方法检测TLR4的表达量,观察益生菌对其表达的影响。结果 (1)益生菌治疗组平均症状改善时间为(9.4±3.2)d,明显短于常规治疗组的(12.6±3.8)d。益生菌治疗组副作用发生率为9.1%,明显低于常规治疗组的27.3%(P<0.05)。(2)伪膜性肠炎患者TLR4表达量为0.732±0.085,明显高于健康对照组0.092±0.020(P<0.01)。经治疗有效后TLR4的表达明显下降,益生菌治疗组下降尤为明显,为0.306±0.057(P<0.05)。结论益生菌能有效治疗伪膜性肠炎,其机制可能与下调TLR4的表达有关。Objective To explore the influence and significance of probiotics on the TLR4 expression in pseudomembranous colonitis.Methods 22 patients with pseudomembranous colonitis were randomly divided into 2 groups： the probiotics group and conventional treatment group.The media time of symptom alleviation and the incidence of side effects were observed.The mononuclearcells were extracted from the peripheral blood of pseudomembranous colonitis patients.The TLR4 expression was detected by RT-PCR and the influence of probiotics on the TLR4 expression was explored.Results（1） The media time of symptom alleviation in probiotics group was（9.4±3.2） days,obviously shorter than that in the conventional treatment group（12.6±3.8） days.And the incidence of side effects in probiotics group was 9.1%,obviously lower than that in the conventional treatment group（27.3%）（P〈0.05）.（2） The TLR4 expression in pseudomembranous colonitis was 0.732±0.085,much higher than that in the health controls 0.092±0.020（P〈0.01）.And its expression was down regulated after treated effectively,especially in probiotics treatment group 0.306±0.057（P〈0.05）.Conclusion The probiotics can effectively treat pseudomembranous colonitis,and the mechanism might be associated with down-regulating TLR4 expression.
