摘 要:目的:探讨骨质疏松症对大鼠胫骨种植体骨结合的影响。方法:将20只老年Wistar雌性大鼠随机分成空白对照组和模型组,模型组采用去势法构建骨质疏松症模型。两组于术前及术后3个月测定大鼠血清碱性磷酸酶(alkaline phosphatase,ALP)水平;术后3个月X线片检测胫骨骨密度及HE染色检查;造模成功后于各组大鼠胫骨近端内植入一枚种植体。术后4周和8周,通过X线片、组织学切片及扫描电镜分析技术检测各组胫骨内种植体骨与周围骨质结合的情况。结果:去势3个月后,模型组血清中ALP水平显著升高,胫骨骨密度降低,骨小梁结构疏松,骨质疏松模型建立成功,而空白对照组无明显变化。种植体植入后4周及8周,空白对照组种植体周围见明显新生骨质形成,结构类似骨皮质,与种植体螺纹相互嵌合,未见明显间隙,骨结合良好。模型组种植体周围见大量疏松结缔组织增生,炎性细胞浸润明显,局部有少量新生骨质形成,结构凌乱不规则,且远离种植体;种植体与周围骨质断裂,间隙明显,骨结合不良。结论:骨质疏松症抑制大鼠胫骨内种植体周围新骨形成,造成种植体骨结合不良。Objective:To investigate the effects of osteoporosis on osteointegration of implant in ovariectomized (OVX) rats. Methods:Twenty female Wistar rats were randomly divided into two groups: Sham and OVX. The serum alkaline phosphatase (ALP) activity was measured pre-and 3-month post operation by an ALP-kit. X-ray and HE staining were taken 3-month post operation. One implant was inserted in the tibia of each rat in Sham and OVX groups. Four and eight-week after implantation, all rats were sacrificed and the tibias with implant were taken by X-ray. The historical changes and the osteointegration of bone and implant were observed by HE stain and SEM scanning. Results:After three months, the right tibia showed decreased bone density and trabecularism porosity en- largement in OVX group, and the serum ALP activity of OVX group was significantly higher than that of before operation( P 〈 0.05 ) , but no change was found in Sham group. New bone formation was determined in the peri-implant area with good osteointegration in Sham group. In OVX group, few new bone formation was found in the peri-implant area and inflammatory reaction and connective tissue was observed between the interface of bone and implant. Conclusions: New bone formation was inhibited in the peri-implant in ovrarie- ctomized rats, which causing inadequacy of bone-implant osteointegration.
