虹膜后粘连性小瞳孔并发白内障的超声乳化治疗疗效观察

摘　　要：目的研究虹膜后粘连瞳孔异常并发白内障行超声乳化摘出人工晶状体植入术的临床疗效。方法对26例30眼虹膜后粘连小瞳孔及瞳孔闭锁并发白内障行超声乳化白内障摘出及人工晶状体植入术,虹膜后粘连患者用黏弹剂冲洗针头分离粘连,撕囊镊撕除瞳孔区机化膜。结果所有病例术后视力较术前有不同程度的提高,术后视力0.1～0.3者13眼(43.33%),0.4以上者l7眼(56.67%);其中0.8以上者6眼,所有患者瞳孔恢复至圆形或近似圆形。主要并发症为角膜水肿、前房纤维素性渗出。结论术前、术中合理的药物应用联合瞳孔区机化膜撕除,原位超声乳化可使虹膜后粘连小瞳孔并发白内障恢复良好的视力和瞳孔形态,无过多手术并发症。Objective To investigate the surgical methods and the clinical efects of phacoemulsification in the state of microcoriacataract. Methods 30 eyes of cataract （26 cases） with small pupil were peformed by using viscoelastic or nonincisional pup - illoplasty first, then underwent stable ultrasonic phacoemulsification. Using the ophthalmic viscoelastic agent and blunt syringe needle to separate the patients, posterior synechia, Tearing the organized membranes on30 eyes of cataract with small pupil. Results All the cases got better visual acuity, the corrected visual acuity being 0.1 -0.3 in 13 eyes （43.33 % ）, above 0.4 in 17 eyes （56.67%） and above 0.8 in 6 eyes. Almost all of the pupils recovered normal or nearly normal. Complications of the surgery included corneal edema, and significant fibrous exudation in the anterior chamber. Conclusion Better clinical outcomes can be achieved after operation,when reasonable surgical techniques and drugs were applied.
