皮下注射吗啡缓解晚期肿瘤顽固呼吸困难的临床研究

摘　　要：目的观察皮下注射吗啡缓解晚期恶性肿瘤顽固呼吸困难的有效率及安全性。方法对20例晚期恶性肿瘤病人出现顽固呼吸困难时给予皮下注射吗啡5mg,必要时2小时后加注1次。采用视觉模拟量表(VAS)和Brog量表评定对呼吸困难的效果,观察各项生命体征指标。结果 VAS评估由治疗前72.3mm降低至治疗后40.1mm,平均减轻(32.2±10.0)mm,差异有显著性(P<0.05)。Brog评分由治疗前7分降至治疗后3分,平均下降(4.0±1.3)分,差异亦有显著(P<0.05)。呼吸频率由治疗前30次/min降低至治疗后23次/min,血氧饱和度由治疗前64.9mmHg升高至治疗后89.8mmHg,心率、血压无明显改变。结论皮下注射吗啡能有效治疗晚期恶性肿瘤患者顽固性呼吸困难,且较安全。Objective To investigate the efficacy and safety of morphine subcutaneous injection for dyspnea appeared in patients with advanced malignant tumor.Methods 0.5 mg of morphine was subcutaneously injected for 20 patients with advanced malignant tumor who had dyspnea.The visua lanalogue scale（VAS） and the Brog scale were adopted to evaluate the efficacy and safety of morphine for dyspnea in advanced tumor.Results VAS decreased from 72.3 mm to 40.1 mm after the treatment,which was significantly reduced as（32.2±10.0） mm（P0.05）.While the Brog scale lowered from 7 pre-treatment to 3 post-treatment,the average reduction was（4.0±1.3） with markable difference（P0.05）.R reduced from 30 to 23 per min,SO2 rose from 64.9mmHg to 89.8mmHg.After s.c.injection of morphine,no obvious change was showed as HR and BP.Conclusion Morphine subcutaneous injection for dyspnea with advanced malignant tumor is safe and effective.
