未成熟网织红细胞指数对肿瘤化疗粒细胞刺激因子预处理的临床意义

摘　　要：目的探讨未成熟网织红细胞指数(IRF)对肿瘤化疗白细胞减少的预测作用,并观察肿瘤患者行粒细胞刺激因子化疗前预处理的疗效。方法采用回顾性研究和前瞻性研究两种方法,分析白细胞减少的恶性肿瘤化疗患者未成熟网织红细胞指数(IRF)和白细胞减少的相关性。在前瞻性研究中,100例肿瘤化疗患者随机分成治疗组50例(重组粒细胞集落刺激因子预处理)和对照组50例(未行重组粒细胞集落刺激因子预处理),在化疗结束后比较两组白细胞减少的发生率。结果未成熟网织红细胞指数(IRF)和白细胞减少呈正相关,高荧光强度网织红细胞比率(HFR%)呈负相关,具有统计学意义(P<0.05)。未成熟网织红细胞指数(IRF)越低,骨髓抑制的发生率越高。恶性肿瘤患者进行粒细胞刺激因子化疗前干预可显著降低白细胞减少发生率(P<0.001)。结论未成熟网织红细胞指数(IRF)可作为评价肿瘤化疗前骨髓造血功能的早期指标,对指导粒细胞集落刺激因子等药物的临床应用有一定的参考价值。Objective To study the clinical significance of the immature reticulocyte fraction and the pretreatment by granulocyte stimulating factor in cancer patients with chemotherapy.Methods We analyzed the correlation between immature reticulocyte fraction and leucopenia in cancer patients with chemotherapy.100 cancer patients were divided into two groups randomly： the treatment group（with the pretreatment by granulocyte stimulating factor） and the control group（without the pretreatment by granulocyte stimulating factor）,then we compared the incidence of the bone marrow suppression.Results The positive correlation was found between immature reticulocyte fraction and leucopenia,and the negative correlation was found between high fluorescence reticulocyte fraction and lecopenia sinificantly,P0.05.The less immature reticulocyte fraction,the higher incidence bone marrow suppression.The pretreatment by granulocyte stimulating factor in cancer patients with chemotherapy could reduce the incidence of bone marrow suppression significantly,P0.05.Conclusion Immature reticulocyte fraction could predict the function of bone marrow suppression in cancer patients with chemotherapy,and help us in guiding the drugs such as granulocyte stimulating factor.
