动脉灌注化疗与全身化疗治疗中晚期胰腺癌疗效对比观察

摘　　要：目的对比动脉灌注化疗与全身化疗治疗中晚期胰腺癌的临床疗效。方法将2010年01月～2011年12月收治诊断明确的中晚期(不能手术)胰腺癌患者68例随机分为对照组和实验组,其中实验组38例给予介入手术(动脉灌注)化疗,对照组30例以相同方案行静脉全身化疗。治疗3周期后,对比两组患者的血肿瘤标志物及临床疗效。结果两组内相比,治疗后所有患者的血CA-199,血CEA变化均优于治疗前(P<0.05);而两组间相比,实验组患者的总有效率为39.5%,高于对照组的30.0%,差异有统计学意义(P<0.05),且血肿瘤标志物变化亦优于对照组(P<0.05)。结论运用介入治疗(动脉灌注化疗)治疗中晚期胰腺癌,临床效果明显优于全身化疗,能够抑制中晚期胰腺癌恶化,并通过一系列生理作用,有效地改善患者的生活质量,对中晚期胰腺癌起到有效地治疗和保护作用。同时动脉灌注化疗的毒副反应低于全身化疗。Objective To compare and analyze the method and effect for interventional treatment（intra-arterial chemotherapy） and systemic chemotherapy in the treatment of advanced pancreatic cancer.Methods 68 cases of a clear diagnosis of advanced（inoperable） pancreatic cancer patients were divided into the control group and the experimental group in accordance with the admission time.The experimental group with 38 cases was taken with interventional treatment,and the control group of 30 cases was taken with the same program of intravenous systemic chemotherapy.After three cycles of treatment,the blood tumor markers and clinical outcome in the two groups were compared.Results Compared in two groups after treatment,the blood of all patients with CA-199,changes in serum CEA was superior to treatment（P0.05）.Compared between the two groups,the total effective rate of the experimental group of patients was 39.5%,30.0% higher than the control group,the difference was statistically significant（P0.05）.The blood tumor marker changes were better than the control group（P0.05）.Conclusion The use of the intervention（intra-arterial chemotherapy） in the treatment of advanced pancreatic cancer,the clinical result is superior to systemic chemotherapy.It can control the deterioration in advanced pancreatic cancer,and by a series of physiological effects,it can improve the quality of life of patients effectively.It plays an effective role in the treatment and protection in patients with advanced pancreatic cancer.
