摘 要:目的探讨下颌角铣骨整形术对矫正下颌角肥大的疗效。方法应用口内切口下颌角铣骨整形术对33例下颌角肥大患者进行矫正。分别于术前、术后3个月行螺旋CT三维重建,观察面部轮廓改善情况。结果33例患者术后3个月复查CT三维重建示,下颌角肥大改善明显,左右对称。下颌角角度由术前均〈1100增大为正常的110°~120°,效果显著。术后33例患者随访6-24个月,非常满意者28例(85%)、满意者5例(15%),无一例发生神经、血管损伤。结论下颌角铣骨整形术,是一种安全有效、操作简便、恢复较快、并发症少的好方法。与截骨术相比,可有效避免“第二下颌角”出现,维持颌颈区立体感。结合CT三维重建,可观察术前、术后下颌角形态,测量下颌角角度,为指导铣骨手术及术后客观评估提供准确参考。Objective To observe the effects of milling-technique in treating mandibular angle hypertrophy. Methods Milling-technique was used to treat mandibular angle hypertrophy of 33 patients through intraoral approach. Three- dimensional reconstruction of CT images preoperatively and postoperative 3 months were applied to observe the facial contour improvement. Results Mandibular angle hypertrophy of 33 patients were improved significantly and bilateral symmetry shown with three-dimensional reconstruction of CT images. The angle of mandibular angle were increased from less than 110 degrees to normally 110-120 degrees postoperatively. 33 patients were satisfied with the contours by following up for 6 to 24 months postoperatively. No severe complications such as injury of blood vessel and nerve observed. Conclusion Milling-technique is a safe, effective and simple method for surgical treatment of mandibular angle hypertrophy. The mandibular angle is grace and second mandibular angle is effectively avoided compared with the mandible osteotomy. Combined With three-dimensional reconstruction of CT images, the counter of mandibular angle can be observed clearly preoperatively and postoperatively to guide and evaluate the milling-technique
