内镜止血联用大剂量洛赛克治疗溃疡病大出血

摘　　要：目的 :探讨内镜止血联用大剂量洛赛克治疗消化性溃疡大出血的临床价值。方法 :对 2 2例溃疡病大出血患者进行内镜治疗 ,然后将内镜止血成功的 2 1例继续抑酸治疗 ,10例用大剂量洛赛克 ,11例用常规剂量西米替丁。结果 :内镜即刻止血成功 2 1例 ,失败 1例。 2 1例内镜止血成功后发生再出血 5例。 10例联用大剂量洛赛克无再出血病例发生。 11例联用西米替丁 ,有 5例发生再出血 ,后经改用大剂量洛赛克治疗后 ,在 48小时内再出血停止。两药再出血率经x2 检验有显著性差异 (P <0 .0 5 )。结论 :内镜止血可迅速缩短溃疡病大出血的病程 ,大剂量洛赛克能迅速提高胃内pH ,持续维持胃高 pH ,保护胃十二指肠粘膜 ,巩固内镜止血疗效 ,防治内镜止血后发生近期再出血。两者联用 ,大大提高了溃疡病人大出血的治愈水平 。Purpose: To explore the clinical value of endoscope hemostasis combine with high does of losec traeatment for peptic ulcer hemorrhea. Methods: 22 cases of peptic ulcer hemorrhea were treated by endoscope. 21 cases of them were succeed in treatment and then given further treatment with acid inhibitor. Among the 21 cases. 10 cases treatment with high does losec and 11 cases with routine does cimetidine. Results: 21 cases succeed in endoscope hemostasis immediatly and 1 case fail. 5 cases were rebleeding after endoscope hemostasis, which is treatment combine with cimetidine, none of which treatment combine with high does losec is rebleeding, and those 5 cases rebleeding were stop in 48 hours after treatment with high does losec. The rebleeding rate of two groups (losec and cimetidine) is significant difference (x 2 test P<0.05). Conclusions: Endoscope hemostasis can rapidly shorten the procedure of peptic ulcer hemorrhea. High does losec can rapidly improve intra gastric pH, maintaining pH in a high level, protecting gastric and duodenal mucosa, reinforcing the efficacy of endoscope hemdstasis and protecting it form current rebleeding. It may be a big promotion of the cure rate of peptic ulcer hemorrhea by endoscope hemostasis combine with high does losec treatment. It is a practial and useful methods in clinical therapy.
