肿瘤标志物联合检测对非小细胞肺癌的诊断价值

摘  要:目的:探讨血清细胞角质蛋白(CYFRA21-1)、组织多肽抗原(TPA)和鳞状细胞癌相关抗原(SCC-Ag)联合检测对非小细胞肺癌(NSCLC)的诊断价值,为临床早期诊断和治疗提供依据。方法:酶联免疫法检测51例非小细胞肺癌(经病理学确诊)、28例小细胞肺癌(经病理学确诊)、77例肺部良性疾病、57例正常人血清的CYFRA21-1、TPA和SCC-Ag的水平,并进行统计学分析。结果:对NSCLC的诊断,CYFRA21-1敏感性最高(72.88%),TPA特异性最高(82.09%),SCC-Ag检测对鳞癌与腺癌的鉴别诊断有统计学意义,三者联合检测NSCLC的敏感性为92.47%,特异性为94.78%,准确率为93.57%。结论:CYFRA21-1、SCC、TPA 3种肿瘤标记物联合检测NSCLC的敏感性及特异性显著高于各独立指标的检测,可以提高NSCLC的早期检出率。Objective:To provide insights of the clinical value of combined measurement of the serum cell keratin(CYFRA21-1),tissue polypeptide antigen(TPA)and squamous carcinoma associated antigen(SCC-Ag)in the diagnosis of non-small cell lung cancer (NSCLC).Methods:The serum levels of CYFRA21-1,TPA and SCC-Ag in 51 NSCLC patients(pathological diagnosis),28 small cell lung cancer patients(pathological diagno- sis),77 benign pulmonary diseases and 57 healthy subjects were measured by enzyme linked immunosorbent assay (ELISA)and the data were analyzed statisticaily,Results:The sensitivity of the CYFRA21-1 to the diag- nosis of NSCLC was 72.88%,and the specificity of the TPA was 82.09%.There was statistical differences be- tween the SCC-Ag value of squamous cell carcinoma and that of adenocarcinoma.Tbe sensitivity of CYFRA21-1,SCC and TPA combined measurement to NSCLC was 92.47% ,the specificity was 94.78% ,and the accuracy was 93.57%.Conclusion:The diagnosis level of CYFRA21-1,SCC,TPA in the sensitivity and specificity are significantly higher than the independent testing.The combined measurement of CYFRA21-1, SCC-Ag and TPA can contribute to improve the early detection rate of NSCLC.
